研究目的：

1. 描述ISD组与透明敷料组在随访90天内肿瘤PICC患者机械性并发症中的导管脱出/异位与堵塞发生率，以及感染性（穿刺点感染、CRBSI）和血栓性（导管相关血栓）并发症的发生情况，并识别各类并发症的主要影响因素； 2. 比较两组在随访90天内复合并发症（机械性、感染性及血栓性）的总体发生率； 3. 调查ISD使用过程中的肿瘤患者导管相关皮肤损伤发生情况；评估护士对ISD操作便捷性、固定稳定性及整体满意度。  

Objectives of Study：

1. To describe the incidence of mechanical complications, including catheter dislocation/misplacement and occlusion, as well as infectious (puncture site infection, CRBSI) and thrombotic (catheter-related thrombosis) complications, in patients with cancer-related PICCs in the ISD group and the transparent dressing group during 90-day follow-up, and to identify the main influencing factors for each complication. 2. To compare the overall incidence of composite complications (mechanical, infectious, and thrombotic) between the two groups during 90-day follow-up. 3. To investigate the incidence of catheter-related skin injuries in cancer patients during ISD use; and to assess nurses' perceptions of ISD ease of use, fixation stability, and overall satisfaction.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1: 18岁及以上，且经临床、影像学或病理学诊断明确为恶性肿瘤，并已制定系统性化疗方案 2: 近一周内于北京协和医院PICC置管室成功置入PICC且确认导管尖端位置正确（上腔静脉下1/3或上腔静脉-右心房交界处） 3: 决策使用PICC作为化疗给药通路 4: 留置PICC后于北京协和医院至少行一次PICC换药 5: 知情同意参与本研究

Inclusion criteria

1.18 years of age or older, with a confirmed diagnosis of malignant tumor by clinical, imaging or pathological examination, and a systemic chemotherapy regimen has been established 2.Successfully placed a PICC in the PICC insertion room of Peking Union Medical College Hospital within the past week and confirmed that the catheter tip is in the correct position (lower 1/3 of the superior vena cava or the superior vena cava-right atrium junction) 3.Decision to use a PICC as a chemotherapy delivery channel 4.At least one PICC dressing change was performed at Peking Union Medical College Hospital after PICC placement 5.Informed consent to participate in this study

1: 皮肤过敏（对敷料、胶带或固定装置已知过敏史） 2: 伴有严重凝血功能障碍 3: 预期生存时间<3个月 4: 接受其他干预性研究者

1.Skin allergy (known history of allergy to dressings, tapes or fixation devices) 2.Accompanied by severe coagulopathy 3.Expected survival time < 3 months 4.Participating in other interventional studies
