研究目的：

该项目的主要目的是比较关节镜下自体髂嵴移植术与 Bankart 联合 Remplissage 治疗前向肩前不稳定伴骨缺损与Hill–Sachs损伤的疗效。次要目的则是评估关节镜下Bankart 联合 Remplissage 治疗前向肩前不稳定伴骨缺损与Hill–Sachs损伤的临床愈合、功能恢复结果。  

Objectives of Study：

The primary objective of this project is to compare the efficacy of arthroscopic autologous iliac crest bone grafting versus Bankart repair combined with Remplissage in the treatment of anterior shoulder instability accompanied by glenoid bone defect and Hill–Sachs lesion. The secondary objective is to evaluate the clinical healing and functional recovery outcomes following arthroscopic Bankart repair combined with Remplissage for anterior shoulder instability with bone defect and Hill–Sachs injury

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）采用关节镜下自体骨移植手术或 Bankart + Remplissage 修复术治疗肩关节前向不稳； （2）随访至少1 年，有可用的CT 或 MRI 资料； （3）关节盂骨缺损1% ~ 35%； （4）伴随 Hill-Sachs 损伤； （5）年龄：16岁 ~ 45岁。

Inclusion criteria

(1)Underwent arthroscopic autologous bone grafting (e.g., iliac crest) or arthroscopic Bankart repair combined with Remplissage procedure for anterior shoulder instability. (2)Minimum follow-up period of 1 year, with available CT or MRI data. (3)Glenoid bone defect ranging from 1% to 35%. (4)Presence of a Hill-Sachs lesion. (5)Age between 16 and 45 years.

排除标准：

（1）多向不稳定性或肩袖损伤； （2）缺乏完整的临床和影像记录，包括临床和手术记录以及失访； （3）既往有肩关节不稳手术史； （4）活动性神经肌肉疾病，例如癫痫活动期。

Exclusion criteria：

(1) Multidirectional instability or rotator cuff injury;? (2) Incomplete clinical and imaging documentation, including missing clinical/surgical records or loss to follow-up;? (3) Previous history of surgical intervention for shoulder instability;? (4) Active neuromuscular disorders, such as active epilepsy.
