研究目的:

本研究目的在于围绕基于 3D Slicer 软件的功能性肺容积测定技术展开探索,解决低肺功能患者 VATS 肺切除术后呼吸功能不全(PORI)预测的关键问题,具体如下:评估功能性肺容积指标对低肺功能患者 VATS 肺切除术后呼吸功能不全(PORI)的预测价值。通过单因素与多因素分析,判断 TE%、TI%、TC% 单独及联合使用时,对患者术后发生 PORI 的预测效能,明确各指标在不同病理类型(如慢性肺气肿、急性肺浸润)患者中的预测适用场景。  

Objectives of Study:

The purpose of this study is to explore the functional lung volume measurement technology based on 3D Slicer software and address the key issues in predicting postoperative respiratory insufficiency (PORI) after video-assisted thoracic surgery (VATS) pulmonary resection in patients with poor pulmonary function. The specific objectives are as follows: to evaluate the predictive value of functional lung volume indicators for PORI in patients with poor pulmonary function following VATS pulmonary resection; to determine the predictive efficacy of TE%, TI% and TC% for postoperative PORI in these patients alone and in combination via univariate and multivariate analyses; and to clarify the applicable predictive scenarios of each indicator in patients with different pathological types (e.g., chronic pulmonary emphysema, acute pulmonary infiltration).

纳入标准:

1.符合PORI诊断标准;
2.年龄在 18 - 75 岁之间,性别不限;
3.因肺部病变已行胸腔镜下肺切除术;
4.术前及术后完成胸部CT检查及常规肺功能检查,图像质量满足三维重建要求;

Inclusion criteria

1.Meeting the diagnostic criteria for postoperative respiratory insufficiency (PORI);
2.Aged 18 to 75 years, with no gender restriction;
3.Having undergone thoracoscopic pulmonary resection for pulmonary lesions;
4.Completing chest computed tomography (CT) scans and routine pulmonary function tests before and after surgery, with image quality meeting the requirements for three-dimensional reconstruction;

排除标准:

1.合并严重慢性阻塞性肺疾病、肺纤维化、肺结核等慢性肺部疾病,影响肺功能评估者;
2.术前已存在呼吸功能不全(PaO?<60mmHg,未吸氧状态)或急性呼吸窘迫综合征;
3.合并严重心脏疾病(如心力衰竭、严重心律失常)、肝肾功能衰竭、凝血功能障碍者;
4.既往肺部手术史或胸部放疗史,影响肺容积测定者;
5.CT图像存在运动伪影、金属伪影等,无法完成三维重建者;

Exclusion criteria:

1.Complicated with severe chronic lung diseases such as chronic obstructive pulmonary disease, pulmonary fibrosis and pulmonary tuberculosis that affect the assessment of pulmonary function; 2.Preoperative presence of respiratory insufficiency (PaO2<60 mmHg, in the non-oxygen inhalation state) or acute respiratory distress syndrome; 3.Complicated with severe heart diseases (e.g., heart failure, severe arrhythmia), hepatic and renal failure, or coagulation dysfunction; 4.Thoracic deformity, a history of previous pulmonary surgery or thoracic radiotherapy that affects the measurement of lung volume; 5.Presence of motion artifacts, metal artifacts and other defects on CT images that prevent the completion of three-dimensional reconstruction;
