研究目的:

本研究旨在利用影像组学方法分析SAH患者的颅脑CT影像,筛选出具有特征性的影像组学指标,并结合血常规、肝功能检测和肾功能检测等血液标志物,构建预测住院期间不良预后的nomogram模型。通过此研究,期望能够提供一种有效的工具,帮助临床医生更准确地评估SAH患者的预后风险,从而优化治疗决策,改善患者的总体预后。  

Objectives of Study:

This study aims to use radiomics methods to analyze cranial CT images of SAH patients, identify radiomic features with characteristic significance, and combine them with blood biomarkers such as routine blood tests, liver function tests, and kidney function tests to construct a nomogram model for predicting adverse outcomes during hospitalization. Through this study, it is hoped to provide an effective tool to help clinicians more accurately assess the prognosis risk of SAH patients, thereby optimizing treatment decisions and improving overall patient outcomes.

纳入标准:

1. 年龄≥18岁,首次急诊入院确诊为蛛网膜下腔出血(SAH)的患者。 2. 入院后24小时内进行颅脑CT扫描。 3. 完整的血常规、肝功能和肾功能检测结果。 4. 出院时有完整的改良Rankin评分(mRS)记录。

Inclusion criteria

1. Age >=18 years, patients admitted to the emergency department for the first time and diagnosed with subarachnoid hemorrhage (SAH). 2. Underwent cranial CT scan within 24 hours of admission. 3. Complete blood count, liver function, and kidney function test results. 4. Complete Modified Rankin Scale (mRS) record at discharge.

排除标准:

1. 既往有脑部外伤或其他神经系统疾病史。 2. 既往有恶性肿瘤或严重的系统性疾病(如慢性心衰、肾衰等)。 3. 入院前接受过相关治疗(如手术、放疗、化疗等)。 4. 影像质量不佳或数据缺失的患者。 5. 难以获得随访数据的患者。

Exclusion criteria:

1. A history of brain injury or other neurological disorders. 2. A history of malignant tumors or severe systemic diseases (such as chronic heart failure, kidney failure, etc.). 3. Patients who received relevant treatments before admission (such as surgery, radiotherapy, chemotherapy, etc.). 4. Patients with poor imaging quality or missing data. 5. Patients for whom follow-up data are difficult to obtain.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

研究数据首先记录于纸质病例报告表(CRF)中。随后由两名经培训的研究人员独立将数据录入至专用的电子采集系统(本研究中采用Excel系统)进行双份录入与核对。若出现数据不一致,需回溯原始病历进行修正。管理过程中建立数据检查轨迹,并在统计分析前完成数据清洗。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data will be initially recorded on Case Report Forms (CRFs). Subsequently, at least two researchers will independently input the data into Excel system to ensure accuracy through double-check and cross-validation. In cases of discrepancies, the original medical records will be retrieved and reviewed for correction. An audit trail will be maintained throughout the data management process, and data cleaning will be completed before statistical analysis.
