研究目的：

已知脑卒中后气管切开是一种常见的急救措施，用于解决因脑卒中导致的呼吸困难和机械通气需求。呼吸功能障碍是卒中后气管切开留置套管患者常见的并发症之一。膈肌作为核心呼吸肌，承担约60%-80%的呼吸肌功能，在静息呼吸状态下贡献约75%-80%的潮气量生成。CPC(ClosedPeripheralCoverage)闭环康复理论首次在2016年由Jia提出，该理论通过结合中枢干预和外周干预，形成一个“闭环”反馈系统，使患者的大脑和肢体在康复过程中形成双向的信息传递与反馈。本课题以闭环理论为基础，使用经颅磁刺激膈肌中枢点联合外周电针刺激刺激天鼎穴、风池穴、颈夹脊穴(C3-C5 节段),电针刺激该穴可以直接刺激呼吸中枢及膈神经，促进呼吸功能的恢复。通过中枢联合外周电针刺激，提高气管切开患者膈肌功能，减少肺部感染，提高拔管效率。  

Objectives of Study：

It is known that tracheotomy after stroke is a common emergency measure used to address dyspnea and the need for mechanical ventilation caused by stroke. Respiratory dysfunction is one of the common complications in patients with indwelling tracheostomy tubes after stroke. As the core respiratory muscle, the diaphragm undertakes approximately 60%-80% of the respiratory muscle function and contributes about 75%-80% of tidal volume generation in the resting breathing state. The CPC (Closed Peripheral Coverage) closed-loop rehabilitation theory was first proposed by Jia in 2016. This theory forms a "closed-loop" feedback system by combining central intervention and peripheral intervention, enabling bidirectional information transmission and feedback between the patient's brain and limbs during the rehabilitation process. Based on the closed-loop theory, this research topic uses transcranial magnetic stimulation on the central point of the diaphragm combined with peripheral electroacupuncture stimulation on Tianding (LI17), Fengchi (GB20), and cervical Jiaji (C3-C5 segments) acupoints. Electroacupuncture stimulation at these acupoints can directly stimulate the respiratory center and phrenic nerve, promoting the recovery of respiratory function. Through central combined with peripheral electroacupuncture stimulation, the diaphragm function of tracheotomy patients can be improved, pulmonary infections can be reduced, and extubation efficiency can be enhanced.

纳入标准：

1.患者诊断符合脑卒中诊断标准; 2.患者首次确诊发病且病程持续时间<=6个月； 3.年龄20~80岁； 4.行气管切开患者; 5.生命体征平稳； 6.能配合完成相关检查及治疗； 7.患者或家属同意并签署知情同意书.

Inclusion criteria

1. The patient meets the diagnostic criteria for stroke; 2. The patient is newly diagnosed with a course of illness <= 6 months; 3. Aged 20–80 years; 4. Undergoing tracheotomy; 5. Vital signs are stable; 6. Able to cooperate with relevant examinations and treatments; 7. The patient or family agrees and signs the informed consent form.

排除标准：

1.患者既往有癫痫史; 2.各种原因不能耐受磁刺激治疗的患者; 3.患者存在重要器官功能严重受损或病情危重； 4.带有心脏起搏器、耳蜗植入或其它植入金属物； 5.严重的认知障碍、情绪障碍、精神异常等不能配合者； 6.患有慢性阻塞性肺疾病、肺结核和肺癌等呼吸系统疾病等。

Exclusion criteria：

1. Patients with a history of epilepsy; 2. Patients who cannot tolerate magnetic stimulation therapy for various reasons; 3. Patients with severe impairment of important organ functions or critical conditions; 4. Patients with a pacemaker, cochlear implant, or other implanted metal devices; 5. Patients with severe cognitive impairment, emotional disorders, or mental abnormalities who cannot cooperate; 6. Patients with respiratory system diseases such as chronic obstructive pulmonary disease, pulmonary tuberculosis, or lung cancer.
