研究目的：

本次试验旨在通过一项双盲随机对照试验，利用床旁超声技术评估，与传统氧气面罩吸氧相比，拔管后应用HFNC能否有效地加快行腹腔镜下全子宫切除术老年患者的膈肌功能恢复。在老年腹腔镜全子宫切除术这一特定人群中，为“拔管后应用HFNC能加快术后膈肌功能恢复”这一假设提供直接证据。  

Objectives of Study：

This trial aimed to evaluate whether the application of high-flow nasal cannula (HFNC) after extubation can effectively accelerate the recovery of diaphragmatic function in elderly patients undergoing laparoscopic total hysterectomy, compared with conventional oxygen mask inhalation, by means of bedside ultrasound technology. It was designed to provide direct evidence for the hypothesis that HFNC administration after extubation can expedite postoperative diaphragmatic function recovery in this specific population of elderly patients undergoing laparoscopic total hysterectomy.

纳入标准：

1.年龄60-74周岁； 2.ASA分级 Ⅰ-Ⅲ 级； 3.计划在全麻下行择期腹腔镜下全子宫切除术； 4.患者知情并签署同意书。

Inclusion criteria

1. Aged 60 to 74 years; 2. American Society of Anesthesiologists (ASA) physical status classification Ⅰ to Ⅲ; 3. Scheduled for elective laparoscopic total hysterectomy under general anesthesia; 4. Provided informed consent and signed the informed consent form.

排除标准：

1.术前存在严重慢性阻塞性肺疾病、重度哮喘、心力衰竭等心肺疾病； 2.体质指数（BMI）≥35 kg/m2； 3.确诊或可疑的神经肌肉疾病或膈肌麻痹病史；； 4.面部或鼻腔解剖异常，无法耐受HFNC或面罩吸氧； 5.急诊手术； 6.拒绝参与实验的患者； 7.采用肺保护通气策略后存在低氧血症风险的患者； 8.PACU停留时间超过2h； 9.有心胸科手术病史患者； 10.手术时间超过3h 。

Exclusion criteria：

1. Preoperative presence of severe cardiopulmonary diseases such as severe chronic obstructive pulmonary disease (COPD), severe asthma, and heart failure; 2. Body mass index (BMI) >= 35 kg/m2; 3. Confirmed or suspected history of neuromuscular disease or diaphragmatic paralysis; 4. Anatomical abnormalities of the face or nasal cavity precluding tolerance of HFNC or oxygen mask inhalation; 5. Emergency surgery; 6. Patients who refuse to participate in the trial; 7. Patients at risk of hypoxemia after implementation of lung-protective ventilation strategy; 8. Length of stay in the post-anesthesia care unit (PACU) exceeding 2 hours; 9. A history of cardiothoracic surgery; 10. Surgical duration exceeding 3 hours.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

在手术前（T0）、进入PACU后接受氧疗之前（T1）以及出PACU时(T2)进行超声下膈肌移动度（DE-QB、DE-DB）评估；在出PACU时记录患者超声下胃窦部横截面积（Cross Sectional Area, CSA），同时采集动脉血气计算氧合指数；在同一时间点进行患者呼吸频率记录；在T0以及术后当天、第一天、第二天、第三天进行Kroenke S-core 评分。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Diaphragmatic excursion (DE-QB, DE-DB) was assessed by ultrasonography before surgery (T0), upon admission to the post-anesthesia care unit (PACU) prior to oxygen therapy (T1) and at discharge from PACU (T2). The cross-sectional area (CSA) of the gastric antrum was measured by ultrasonography at discharge from PACU, and arterial blood gas samples were collected simultaneously to calculate the oxygenation index. The respiratory rate of patients was recorded at the aforementioned time points. The Kroenke Score was evaluated before surgery (T0) and on the day of surgery, the 1st, 2nd and 3rd postoperative days.
