研究目的：

采用“目标试验模拟”框架，在合并2型糖尿病且行R0肝切除的肝细胞癌患者中，评估术后启动GLP-1RA 相比启动DPP-4i 对无复发生存期和总生存期的影响。  

Objectives of Study：

To estimate, within a target-trial emulation framework, the effect of initiating GLP-1 receptor agonists (GLP-1RA) versus DPP-4 inhibitors (DPP-4i) on recurrence-free survival (RFS) and overall survival (OS) after R0 hepatectomy among adults with type 2 diabetes and histologically confirmed hepatocellular carcinoma (HCC).

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 18岁以上的成人。 2.组织学确诊的肝细胞癌(HCC)且已行R0切除。 3.手术前或手术当日已记录的2型糖尿病。 4. 在手术后0-90天内记录了GLP-1RA或DPP-4i的首次合格处方。 5.根据既往医疗史和用药史，患者此前未曾使用过 GLP-1RA 或 DPP-4i药物。

Inclusion criteria

1. Adults over 18 years old. 2. Histologically confirmed hepatocellular carcinoma (HCC) and with R0 resection performed. 3. Type 2 diabetes that was recorded either before or on the day of the surgery. 4. The first qualified prescription of GLP-1RA or DPP-4i was recorded within 0-90 days after the surgery. 5. The patient had not previously used GLP-1RA or DPP-4i drugs based on their previous medical history and medication record.

排除标准：

1.非HCC病理学特征、R1/R2 切除范围，或根据影像学检查、手术发现及/或病理学结果所显示的术中或术前存在大血管侵犯的证据。 2.既往肝移植史。 3.1型糖尿病或胰腺源性糖尿病，或在首次合格医嘱/处方前有记录的急性胰腺炎。 4.在确认窗口内，当日开具GLP-1RA和DPP-4i的双类处方。 5. 关键数据质量问题，导致无法可靠分配或结果判定。

Exclusion criteria：

1. Absence of HCC pathological features, R1/R2 resection range, or evidence of major vascular invasion during or before the operation as indicated by imaging examinations, surgical findings, and/or pathological results. 2. History of liver transplantation. 3. Type 1 diabetes or pancreatic diabetes, or documented acute pancreatitis before the first qualified prescription. 4. Administration of dual prescriptions of GLP-1RA and DPP-4i on the same day within the confirmation window. 5. Key data quality issues that prevent reliable allocation or result determination.
