研究目的：

探讨比较常规液体输注组与 IVC-CI 导向液体管理组对老年无痛胃肠镜诊疗低血压及恢复质量的影响是否存在差异  

Objectives of Study：

To investigate whether there are differences between conventional fluid infusion and IVC-CI guided fluid management in their effects on hypotension and recovery quality in elderly patients undergoing painless gastrointestinal endoscopy

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.自愿签署知情同意书； 2.年龄 ≥ 65 岁； 3.拟择期行无痛胃肠镜诊疗者； 4.ASA 分级Ⅱ~Ⅲ级； 5.术前禁食 ≥ 8 小时，禁水 ≥ 4 小时。

Inclusion criteria

1,Voluntarily sign the informed consent form; 2.Age >= 65 years; 3.Scheduled for elective painless gastrointestinal endoscopy; 4.ASA physical status classification II-III; 5.Preoperative fasting for >= 8 hours and clear fluid restriction for >= 4 hours.

排除标准：

1.心功能不全（NYHA Ⅲ ~ Ⅳ级）； 2.预估为困难气道者； 3.消化道梗阻、胃排空延迟或反流高风险； 4.言语沟通障碍者、老年痴呆者； 5.肠道准备不充分者； 6.下腔静脉超声无法清晰显示者； 7.正在参加另一项干预性临床研究。

Exclusion criteria：

1.Cardiac insufficiency (NYHA class III–IV); 2.Anticipated difficult airway; 3.Gastrointestinal obstruction, delayed gastric emptying, or high risk of reflux; 4.Communication disorders or dementia; 5.Inadequate bowel preparation; 6.Inability to obtain a clear ultrasonographic view of the inferior vena cava; 7.Current participation in another interventional clinical trial.

VC-CI 导向输液：IVC-CI > 40%，5 ~10 min内输 3 ml/kg，输液完成后 5 min 再次测量 IVC-CI；IVC-CI20% ~40%，给予 4 ml/kg·h ；IVC-CI≤20%，给予2kml/kg·h。

Fluid administration protocol guided by IVC-CI: If IVC-CI > 40%: Administer a 3 ml/kg bolus over 5-10 minutes. Re-measure IVC-CI 5 minutes after completion of the fluid bolus. If IVC-CI is 20% to 40%: Administer a maintenance infusion at 4 ml/kg/h. If IVC-CI <= 20%: Administer a maintenance infusion at 2 ml/kg/h.
