研究目的：

观察30°半卧位对PACU内腹腔镜胆囊术后低氧血症发生率的影响，及其对PACU停留时间以及术后肺部并发症的影响，并用无创膈肌超声观察膈肌移动度提供客观依据，为此类患者PACU内管理提供理论依据和临床资料,优化此类患者的恢复期管理，提供一种简便易行的方法，提高围术期安全性。  

Objectives of Study：

To investigate the effect of a 30° semi-recumbent position on the incidence of postoperative hypoxemia in the Post-Anesthesia Care Unit (PACU) following laparoscopic cholecystectomy, as well as its impact on PACU length of stay and postoperative pulmonary complications. Additionally, to utilize non-invasive diaphragmatic ultrasound to observe diaphragmatic excursion, thereby providing objective evidence. This study aims to furnish a theoretical basis and clinical data for the management of such patients in the PACU, optimize their recovery phase management, offer a simple and feasible method, and enhance perioperative safety.

纳入标准：

1.年龄18-65岁之间； 2.ASAⅠ- Ⅱ级； 3.体重指数(BMI)18-28 kg/m2； 4.拟在气管插管全身麻醉下接受择期腹腔镜胆囊手术的患者； 5.理解方案并自愿签署知情同意书者；

Inclusion criteria

1.Aged between 18 and 65 years. 2.American Society of Anesthesiologists (ASA) physical status classification I or II. 3.Body Mass Index (BMI) between 18 and 28 kg/m2. 4.Scheduled to undergo elective laparoscopic cholecystectomy under endotracheal general anesthesia. 5.Capable of comprehending the study protocol and voluntarily providing written informed consent.

排除标准：

1.膈肌功能障碍或神经肌肉疾病； 2.胸廓畸形； 3.气胸，肺结核，胸腔积液，COPD，哮喘，OSAHS，既往肺部手术； 4.预计插管困难的病例； 5.术前大气压下SpO2＜95%； 6.既往有神经系统疾病不能主动合作，无法获得临床资料者； 7.对于所使用的麻醉药物存在过敏反应的患者； 8.患有青光眼； 9.怀孕、哺乳； 10.术前严重贫血患者，Hb＜90g/L；

Exclusion criteria：

1.Diaphragmatic dysfunction or neuromuscular disorders. 2.Thoracic deformity. 3.Conditions including pneumothorax, pulmonary tuberculosis, pleural effusion, chronic obstructive pulmonary disease (COPD), asthma, obstructive sleep apnea-hypopnea syndrome (OSAHS), or history of previous pulmonary surgery. 4.Anticipated difficult airway/intubation. 5.Preoperative pulse oximetry (SpO?) reading of < 95% at room air. 6.History of neurological disorders precluding active cooperation or compromising the acquisition of clinical data. 7.Known hypersensitivity or allergic reaction to any of the anesthetic agents to be used in the study. 8.Glaucoma. 9.Pregnancy or lactation. 10.Preoperative severe anemia, defined as hemoglobin (Hb) level < 90 g/L..
