研究目的：

1.明确rTMS联合ASMs治疗对EE-SWAS患儿ESES现象、癫痫发作、认知损伤的近期和远期治疗效果； 2.明确rTMS治疗在EE-SWAS患儿中的安全性和耐受性，为儿童EE-SWAS的临床治疗提供依据和参考。  

Objectives of Study：

1.To determine the short- and long-term therapeutic effects of rTMS combined with ASMs on ESES phenomena, seizure frequency, and cognitive impairment in children with EE-SWAS; 2.To assess the safety and tolerability of rTMS treatment in children with EE-SWAS, providing evidence and references for the clinical management of pediatric EE-SWAS.

纳入标准：

1.年龄3-14岁； 2.EE-SWAS满足 2022 年国际抗癫痫病协会（ILAE）癫痫综合征诊断标准； 3.同意参加本研究并签署知情同意书； 4.能够按时完成视频脑电图、rTMS治疗和神经认知发育评估等检查。

Inclusion criteria

1. Age 3-14 years; 2. EE-SWAS meets the 2022 diagnostic criteria for epilepsy syndromes set by the International League Against Epilepsy (ILAE); 3. Agree to participate in this study and sign the informed consent form; 4. Able to complete tests such as video EEG, rTMS treatment, and neurocognitive development assessment on schedule.

排除标准：

1.存在 rTMS 刺激禁忌：头颅内置有金属异物者、带心脏起搏器者、有耳蜗植入物者等，即不能接近电磁铁的患儿； 2.缺少视频脑电图和神经认知发育评估检查资料； 3.存在现症感染的依据（如各种类型的疱疹病毒、人类免疫缺陷病毒、肝炎病毒、梅毒等感染）； 4.严重心、肺、肾、肝、骨髓功能障碍者等。

Exclusion criteria：

1. Contraindications for rTMS stimulation: children with metal implants in the skull, those with a pacemaker, cochlear implants, etc., meaning they cannot be near an electromagnet; 2. Lack of video EEG and neurocognitive development assessment data; 3. Evidence of current infections (such as various types of herpes viruses, HIV, hepatitis viruses, syphilis, etc.); 4. Severe dysfunction of the heart, lungs, kidneys, liver, or bone marrow.

在根据针对病情选择设施的抗癫痫药物治疗的基础上，采用北京华星康泰科技发展有限公司生产的HX-C3儿童专用型rTMS治疗仪，采用环形线圈，根据VEEG 在Rolandic区放电优势位置确定刺激部位；治疗时帮助患儿取得舒适体位．安抚患儿情绪，指导其下颚放置在固定支架上。线圈放置在与头皮相平行的上方。治疗参数：刺激频率为1Hz，刺激时间为2s，间歇时间为8s，每次20min，周一至周六，每日1次，周日休息。

干预措施代码：

Intervention：

Based on the individualized anti-seizure medications regimen tailored to the specific clinical condition, the HX-C3 pediatric-specific rTMS therapeutic apparatus produced by Beijing Huaxing Kangtai Technology Development Co., Ltd. was used. A circular coil was employed, with the stimulation site determined based on the dominant discharge location in the Rolandic area as identified by VEEG. During treatment, the child was assisted in achieving a comfortable position, their emotional state was soothed, and they were guided to place their chin on a fixed support. The coil was positioned parallel to and above the scalp. Treatment parameters: stimulation frequency of 1 Hz, stimulation duration of 2 s, inter-stimulus interval of 8 s, each session lasting 20 minutes, administered once daily from Monday to Saturday, with rest on Sunday.
