研究目的：

本研究的目的是应用二元应对理论指导女性不孕症患者配偶共同参与方案并进行有效性评价。从病耻感及心理痛苦两项结局指标对该方案的可行性与实用性进行验证，为持续改进女性不孕症患者心理健康提供参考依据。  

Objectives of Study：

The purpose of this study was to apply dyadic coping theory to guide spouses of female infertility patients to participate in the program together and evaluate its effectiveness. The feasibility and practicability of the program were verified from the two outcome indicators of stigma and psychological distress, so as to provide a reference for continuous improvement of the mental health of female infertility patients.

纳入标准：

1.行辅助生殖技术助孕的的女性不孕症患者； 2.语言功能正常，能正确表达自己的想法； 3.知情同意，愿意参加本研究。

Inclusion criteria

1. Female infertility patients who underwent assisted reproductive technology; 2. Normal language function, can correctly express their own ideas; 3. Informed consent and willingness to participate in this study.

排除标准：

1.合并有严重心、脑、肺等脏器功能不全者； 2.有重大残疾或精神疾病史者。

Exclusion criteria：

1. Combined with severe heart, brain, lung and other organ dysfunction; 2. With a history of major disability or mental illness.

对照组患者进行常规教育，包括：1.健康宣教：对不孕患者详细讲解不孕症的发病原因及治疗方法等知识，提高患者的疾病认知度，诊疗各个环节为患者和家属答疑解惑；2.饮食指导：对患者生活和饮食进行指导，根据患者营养状态和口味习惯制订食谱，告知患者规范饮食的重要性引导患者合理选择饮食方案和烹饪方式，以满足营养需求；3.运动指导：督促其保持良好的生活习惯，并适当进行体育锻炼，嘱其避免过度劳累，保证睡眠质量；4.心理支持：对患者进行心理辅导和支持，主动与患者沟通，了解其心理状况，帮助其缓解消极情绪。

The patients in the control group received routine education, including: 1. Health education: explaining the causes and treatment methods of infertility in detail to improve the patient's awareness of the disease, and answering questions for patients and their families in each link of diagnosis and treatment; 2. Diet guidance: guide patients' life and diet, formulate recipes according to patients' nutritional status and taste habits, inform patients about the importance of standardized diet and guide patients to choose a reasonable diet plan and cooking methods to meet their nutritional needs; 3. Exercise guidance: urging them to maintain good living habits, and appropriate physical exercise, told them to avoid overwork and ensure the quality of sleep; 4. Psychological support: provide psychological counseling and support to patients, actively communicate with patients, understand their psychological status, and help them relieve negative emotions.

干预措施：

干预组在接受对照组干预的基础上，采用配偶共同参与方案进行干预。（1）第一次干预：患者-配偶认知共同重建。在建档当天，由研究护士向患者与配偶讲解IVF-ET 治疗流程、不孕症的医学成因，纠正配偶与患者对不孕症的认知偏差，减少因“疾病标签化”引发的病耻感。（2）第二次干预：配偶支持技能培训。促排卵当天，研究护士通过案例讲解，帮配偶识别患者病耻感的典型表现；培训配偶“共情式沟通话术”，避免无效安慰。（3）第三次干预：配偶陪伴与身体照护干预。在促排卵、取卵、移植等关键节点，通过配偶的尽量陪伴就诊与生活上的主动协助，减少患者的孤独感与自我污名。（4）第四次干预：配偶参与的期待管理。针对移植后“等待验孕”的焦虑期，通过配偶的积极引导，减少患者因担心失败引发的自我污名化。

Intervention：

The intervention group received the intervention of the control group on the basis of the intervention of the spouse participation program. (1) The first intervention: patient-spouse cognitive reconstruction. On the day of filing, research nurses explained the IVF-ET treatment process and the medical causes of infertility to the patients and their spouses, corrected the cognitive bias of spouses and patients on infertility, and reduced the stigma caused by "disease labeling". (2) The second intervention: spousal support skills training. On the day of ovulation induction, the research nurse helped the spouse identify the typical manifestations of the patient's stigma through case lectures. Empathic communication skills should be trained to avoid ineffective comfort. (3) The third intervention: spouse companionship and physical care intervention. At the key nodes such as ovulation induction, oocyte retrieval and transplantation, the patient's loneliness and self-stigma can be reduced through the companionship of the spouse and active assistance in life. (4) The fourth intervention: expectant management with the participation of spouses. For the anxiety period of "waiting for pregnancy test" after transplantation, the positive guidance of the patient's spouse was used to reduce the self-stigma caused by the fear of failure.
