研究目的：

验证社会生态模型下 2 型糖尿病病人自我管理干预方案改善社区T2DM 病人社会资源利用、自我管理行为、空腹血糖（ FPG）、糖化血红蛋白（ HbAlc）、低密度脂蛋白胆固醇（ LDL）、血压（ BP）、体质指数（ BMI）及生活质量的效果。  

Objectives of Study：

To test the effects of a self-management intervention program based on social-ecology model on social resource utilization,self-management behavior, blood glucose, blood pressure(BP), body mass index(BMI) and quality of life(QofL) among type 2 diabetes mellitus patients(T2DM).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合 WHO1999 年制定的 T2DM 诊断标准，经医生确诊；
②年龄 18-75 岁；
③无沟通障碍；
④有中文读、写能力；
⑤为社区卫生服务站登记的本地常住人口，居住时间>1 年；
⑥自愿参与本研究。

Inclusion criteria

1. meet WHO1999 T2DM diagnostic criteria and confirmed by doctors;
2. ages 18-75 years;
3. no communication barriers;
4. able to read and write in Chinese;
5. residence registered in the community health service station, whose residence time was more than 1 year;
6. Volunteer to participate in this study.

排除标准：

①合并严重并发症(增殖性视网膜病、肾病Ⅳ期及以上、心功能Ⅲ级及以上、糖尿病足 1 级及以上)；
②合并其他严重慢性病如脑卒中、恶性肿瘤等；
③近半年有长期外出居住计划。

Exclusion criteria：

1. with severe complications (hyperplastic retinopathy, nephropathy in period IV or more, cardiac function level III or more, and diabetic foot level 1 or more;
2. with other serious chronic diseases such as stroke, malignant tumors;
3. have a long-term outside living plan within nearly half a year.
