研究目的：

评价在严格筛选的单侧肾母细胞瘤患儿中，应用保留肾单位手术对比历史数据中的肾切除，在主要肿瘤学预后上是否达到非劣效。以3年OS和3年EFS作为核心衡量指标。研究设计为前瞻性队列，并采用历史对照的非劣效性检验回应了保留肾单位手术在单侧肾母细胞瘤中肿瘤学安全性的核心争议;研究的主要评估终点设定为治疗结束后3年。  

Objectives of Study：

Evaluation of whether nephron-sparing surgery (NSS) achieves non-inferior major oncological outcomes compared to historical nephrectomy data in strictly selected children with unilateral Wilms tumor, with 3-year overall survival (OS) and 3-year event-free survival (EFS) as the core efficacy measures. The study is designed as a prospective cohort with historical controls, employing a non-inferiority test to address the core controversy regarding the oncological safety of NSS in unilateral Wilms tumor. The primary evaluation endpoint is set at 3 years after completion of treatment.

纳入标准：

1. 年龄 > 6个月； 2.临床或组织学诊断为WT； 3. 患儿未接受过任何形式的肿瘤切除手术； 4. 心、肝、肺、肾等重要脏器功能可满足化疗和手术要求； 5. 获得患者监护人的书面知情同意。

Inclusion criteria

1. Age > 6 months; 2. Clinical or histological diagnosis of WT; 3. The patient has not undergone any form of tumour resection surgery; 4. Vital organ function (heart, liver, lungs, kidneys, etc.) is sufficient to meet the requirements of chemotherapy and surgery; 5. Written informed consent has been obtained from the patient's guardian.

排除标准：

1. 年龄 > 18岁； 2. 组织学诊断为非WT的肾肿瘤； 3.存在可能阻碍本研究进行的严重合并症（如需透析的肾功能损害、肝功能衰竭、心功能衰竭等）； 4.因家属原因等导致不能完成本研究方案规定的流程。

Exclusion criteria：

1. Age > 18 years; 2. Histological diagnosis of non-Wilms' tumour renal neoplasm; 3. Presence of severe comorbidities that may impede study conduct (e.g., renal impairment requiring dialysis, hepatic failure, cardiac failure); 4. Inability to complete the study protocol due to family circumstances or other reasons.
