研究目的：

本研究旨在探讨IBD患者的膳食模式及生活方式与营养状况、疾病活动度的关联，为营养与生活方式干预提供依据。  

Objectives of Study：

This study aims to investigate the association between dietary patterns and lifestyle factors in patients with inflammatory bowel disease (IBD) and their nutritional status and disease activity, thereby providing evidence for nutrition and lifestyle interventions.

纳入标准：

1.年龄>=18周岁；
2.根据国际共识（如ECCO），经临床表现、内镜检查、影像学检查和/或组织病理学检查确诊为克罗恩病（CD）或溃疡性结肠炎（UC）；
3.意识清晰，具备基本的读写或沟通能力，能够理解研究内容并独立或在协助下完成问卷调查；
4.自愿参加本研究并签署书面知情同意书。

Inclusion criteria

1.Age >=18 years;
2.Diagnosed with Crohn's disease (CD) or ulcerative colitis (UC) based on clinical presentation, endoscopy, imaging studies and/or histopathological examination in accordance with international consensus guidelines (e.g. ECCO);
3.Conscious and lucid, possessing basic literacy or communication skills sufficient to comprehend the study content and complete questionnaires independently or with assistance;
4.Voluntarily participating in this study and having signed a written informed consent form.

排除标准：

1.合并有其他严重影响营养代谢或消化吸收的疾病（如：恶性肿瘤、严重肝肾功能不全（Child-Pugh C级）、慢性心力衰竭（NYHA III-IV级）、未控制的甲状腺疾病）；
2.处于妊娠或哺乳期的女性；
3.在过去3个月内接受过肠道外科手术（为避免术后短期营养和饮食的剧烈波动干扰）；
4.存在严重的认知或精神障碍，无法配合完成调查；
5.拒绝签署知情同意书。

Exclusion criteria：

1.Concurrent severe conditions significantly impacting nutritional metabolism or digestive absorption (e.g., malignant tumours, severe hepatic or renal impairment [Child-Pugh Class C], chronic heart failure [NYHA Class III-IV], uncontrolled thyroid disorders);
2.Women who are pregnant or breastfeeding;
3.Individuals who have undergone intestinal surgery within the past three months (to avoid interference from significant short-term fluctuations in nutrition and diet post-surgery);
4.Those with severe cognitive or psychiatric disorders rendering them unable to cooperate with the study;
5.Individuals who refuse to sign the informed consent form.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 病例记录表（CRF）：采用统一设计的纸质及电子病例报告表（版本号：1.0，日期：2025.11.24），内容涵盖研究对象基本信息与社会人口学特征、疾病临床特征（IBD 类型、确诊时间、肠外表现、疾病活动度评分等）、膳食摄入评估（食物频率问卷）、生活方式评估（体力活动、睡眠、压力量表）、营养状况评估（人体测量、生化指标）、患者报告结局与生活质量（IBD 生活质量问卷）等核心研究数据，确保数据采集的标准化与完整性； 2. 电子采集和管理系统（EDC）：eCRF 系统部署在郑州大学第二附属医院内网服务器，通过平板电脑或诊室电脑直接录入数据，系统设有强制字段和逻辑核查功能（如疾病活动度缓解期时特定症状字段自动隐藏或要求复核），保障数据完整性与准确性；同时执行数据安全管理，包括为每位参与者生成唯一研究识别码（如 “ZZU2F-IBD-001”），个人身份信息与研究数据分离加密存储，每日自动备份，且仅限授权研究人员访问

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Case Record Form (CRF): A standardized paper and electronic Case Report Form (Version 1.0, Date: 2025.11.24) is used, covering core research data such as participants' basic information and sociodemographic characteristics, clinical features of the disease (IBD type, diagnosis time, extraintestinal manifestations, disease activity score, etc.), dietary intake assessment (Food Frequency Questionnaire), lifestyle assessment (physical activity, sleep, stress scales), nutritional status assessment (anthropometry, biochemical indicators), and patient-reported outcomes and quality of life (IBD Quality of Life Questionnaire), ensuring the standardization and completeness of data collection; 2. Electronic Data Capture (EDC): The eCRF system is deployed on the internal network server of the Second Affiliated Hospital of Zhengzhou University. Data is directly entered through tablets or clinic computers. The system is equipped with mandatory fields and logical verification functions (e.g., specific symptom fields are automatically hidden or require review when the disease activity is in remission) to ensure data integrity and accuracy. At the same time, data security management is implemented, including generating a unique research identification code (e.g., "ZZU2F-IBD-001") for each participant, separating and encrypting the storage of personal identification information and research data, automatic daily backup, and access restricted to authorized researchers
