研究目的：

本项目拟在前期设计研发并进行了初期性能验证的下颌骨骨折快速外固定支架的基础上，通过小样本临床试验进行扩试研究完成下颌骨骨折快速外固定支架的效果验证，最终形成待转化推广的成熟下颌骨骨折快速外固定支架产品，有效提升下颌骨粉碎性骨折损伤控制能力，并进一步降低受伤后遗留面部畸形的重建难度。  

Objectives of Study：

Based on the previously designed, developed, and preliminarily performance-verified rapid external fixation device for mandibular fractures, this project intends to carry out expanded research through small-sample clinical trials to complete the efficacy verification of the rapid external fixation device for mandibular fractures. Ultimately, a mature rapid external fixation device product for mandibular fractures, ready for transformation and promotion, will be developed, effectively improving the control ability for comminuted mandibular fracture injuries and further reducing the difficulty of reconstructing facial deformities left after injury.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.粉碎性骨折。下颌骨因车祸等原因导致的开放性或伴有感染的严重骨折；下颌骨骨折患者同时并发颅脑损伤、气胸、腹部脏器损伤无法进行专科治疗的患者；或专科情况尚可，因严重高血压、急性脑梗塞、甲状腺功能亢进症控制不稳定等原因不能即刻行下颌骨切开复位内固定的患者； 2.骨髓炎等相关疾病。进行下颌骨重建修复术需要术中临时固定的患者；或感染严重导致病理性骨折，如因下颌骨骨髓炎、药物性骨坏死、放射性骨坏死等炎症性疾病导致下颌骨破坏伴发严重感染，不适宜即刻行颌骨部分切除术+同期骨移植或假体修复进行下颌骨重建或需重建板临时固定的患者； 3.肿瘤等原因导致的下颌骨缺损。下颌骨因肿瘤等原因进行下颌骨重建修复术需要术中临时固定的患者；或需行节段性下颌骨截除术无法进行现阶段腓骨、髂骨等自体骨移植或替代假体修复的患者或原有假体出现感染等情况的患者； 4.患者完全知情本研究项目的内容、受益和可能承担的风险，并同意加入本项目研究的患者； 5.可正常沟通与交流，并能够积极配合者。

Inclusion criteria

1.Comminuted fractures. Severe open or infected fractures of the mandible caused by traffic accidents or other reasons; patients with mandibular fractures who also have concomitant craniocerebral injury, pneumothorax, or abdominal organ injury that make specialized treatment impossible; or patients whose specialized condition is acceptable but cannot undergo immediate open reduction and internal fixation of the mandible due to reasons such as severe hypertension, acute cerebral infarction, or unstable control of hyperthyroidism; 2.Related diseases such as osteomyelitis. Patients requiring temporary intraoperative fixation during mandibular reconstruction and repair surgery; or patients with severe infection leading to pathological fractures, such as those with destruction of the mandible accompanied by severe infection caused by mandible osteomyelitis, medication-induced osteonecrosis, or radiation-induced osteonecrosis and other inflammatory diseases, who are not suitable for immediate partial mandibulectomy with simultaneous bone grafting or prosthetic repair for mandibular reconstruction, or who require temporary fixation with a reconstruction plate; 3.Mandibular defects caused by tumors or other reasons. Patients requiring temporary intraoperative fixation during mandibular reconstruction and repair surgery due to tumors or other causes; or patients requiring segmental mandibulectomy who are unable to undergo autologous bone grafts such as fibula or iliac bone transplantation, or alternative prosthetic repair at this stage, or patients with existing implants experiencing infection or other issues; 4.Patients who are fully informed of the content, benefits, and possible risks of this research project, and agree to participate in this study; 5.Patients who can communicate and interact normally, and are able to cooperate actively.

排除标准：

1.具备手术切开复位行坚固内固定治疗条件的患者； 2.下颌骨缺损范围导致双侧升支均受累，无法进行单侧外固定装置固定的患者； 3.患有癫痫或精神类疾病不能配合术后管理的患者； 4.患者不愿意承担本项目可能带来的风险，进而无法满足随访需求的患者； 5.无完整且真实临床资料者。

Exclusion criteria：

1.Patients who meet the conditions for surgical open reduction and rigid internal fixation treatment; 2.Patients whose mandibular defect range involves both ascending rami, making it impossible to fix with a unilateral external fixation device; 3.Patients with epilepsy or mental disorders who are unable to cooperate with postoperative management; 4.Patients who are unwilling to bear the potential risks of this project, and thus cannot meet the follow-up requirements; 5.Patients without complete and authentic clinical data.
