研究目的：

探讨血清铁蛋白（SF）与简易胰岛素抵抗替代指标（TyG指数、TG/HDL-C指数、TyG-BMI指数、HOMA-IR）两两联合对2型糖尿病（T2DM）患者早期肾损伤（UACR≥30 mg/g）的交互作用及联合预测价值  

Objectives of Study：

To investigate the interaction and combined predictive value of serum ferritin (SF) paired with each simple surrogate index of insulin resistance—TyG index, TG/HDL-C ratio, TyG-BMI index, and HOMA-IR—for early renal injury (UACR >= 30 mg/g) in patients with type 2 diabetes mellitus (T2DM).

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄40-70岁，男女不限；2.根据《中国糖尿病防治指南(2024 版)》糖尿病诊断与分型标准确诊为 2 型糖尿病的患者；3.临床资料完整；4.愿意并签署知情同意书者。

Inclusion criteria

1. Aged 40-70 years, either sex. 2. Diagnosed with type 2 diabetes mellitus according to the diagnostic and classification criteria in the "Chinese Guidelines for the Prevention and Treatment of Diabetes Mellitus (2024 Edition)". 3. Complete clinical data available. 4. Willing to participate and has signed informed consent.

排除标准：

1.1型糖尿病患者，其他特殊类型糖尿病患者；2.有高血压病史；
3.有肾脏病史的患者，包括急、慢性肾炎，尿路感染等；4.中、晚期肾脏疾病患者；5.合并糖尿病急性并发症如糖尿病酮症酸中毒、高渗高血糖状态；6.合并贫血；
7.研究前1个月使用过胰岛素、免疫制剂、激素、铁剂、抗炎药物的患者；8.有自身免疫性疾病；9.有严重的全身性疾病如严重感染、严重的心肝肾功能损害（心衰、尿毒症、血清总胆红素＞171μmol/L，或直接胆红素＞34μmol/L；凝血酶原活动度＜40%；血清白蛋白＜25g/L；出现肝性脑病、肝肾综合征、严重腹水等并发症，或有明显的出血倾向；Scr＞707μmol/L，BUN＞28.6mmol/L）、严重水、电解质和酸碱平衡紊乱（如血钾＞6.5mmol/L、代谢性酸中毒pH＜7.2）、恶性肿瘤等疾病的患者；10.妊娠期、哺乳期或长期服用避孕药的妇女。

Exclusion criteria：

1. Subjects with type 1 diabetes or any other specific types of diabetes. 2. Individuals with a history of hypertension. 3. Patients with any renal disease, including acute or chronic glomerulonephritis, urinary-tract infection, etc. 4. Subjects with moderate-to-advanced chronic kidney disease. 5. Presence of acute diabetic complications such as diabetic ketoacidosis or hyperosmolar hyperglycaemic state. 6. Concomitant anaemia. 7. Use of insulin, immunosuppressants, corticosteroids, iron preparations, or anti-inflammatory drugs within the past month. 8. Autoimmune disorders. 9. Severe systemic illnesses: serious infection;
1.marked impairment of cardiac, hepatic or renal function (e.g. heart failure, uraemia, total bilirubin >171 μmol/L or direct bilirubin >34 μmol/L, prothrombin activity <40%, serum albumin <25 g/L, hepatic encephalopathy, hepatorenal syndrome, intractable ascites, overt bleeding tendency);
2.Scr >707 μmol/L and/or BUN >28.6 mmol/L;
3.severe water–electrolyte or acid–base disturbances (e.g. serum potassium >6.5 mmol/L, metabolic acidosis with pH <7.2);
4.malignancy. 10. Pregnant or lactating women, or women taking long-term oral contraceptives.
