研究目的：

1. 探讨术中房颤状态下及消融后窦性状态下的左房压参数（收缩压、舒张压、平均压及脉压差）与持续性房颤患者导管消融术后远期复发的关系； 2. 比较房颤与窦性下左房压联合左房基质对房颤术后远期的预测价值； 3. 确定预测房颤消融术后复发的最佳左房压参数及其临界值； 4. 评估左房压参数与传统预测因子（如左房直径、房颤持续时间等）的独立关联性； 5. 验证超声心动图无创指标（E/e'比值、左房容积指数）与有创左房压的相关性，比较其预测消融复发的效能。  

Objectives of Study：

1. To investigate the relationship between intra-procedural left atrial pressure parameters (systolic, diastolic, mean pressures, and pulse pressure) measured during atrial fibrillation and after restoration of sinus rhythm following ablation, and the long-term recurrence after catheter ablation in patients with persistent atrial fibrillation. 2. To compare the predictive value for long-term post-ablation recurrence of left atrial pressure combined with left atrial substrate characteristics between the atrial fibrillation and sinus rhythm states. 3. To identify the optimal left atrial pressure parameter(s) and its/their cut-off value(s) for predicting recurrence after atrial fibrillation ablation. 4. To assess the independent association of left atrial pressure parameters with traditional predictors (e.g., left atrial diameter, duration of atrial fibrillation). 5. To validate the correlation between non-invasive echocardiographic indices (E/e' ratio, left atrial volume index) and invasive left atrial pressure measurements, and to compare their predictive efficacy for ablation recurrence.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.持续性房颤（持续时间大于7天且小于1年）或长期持续性房颤（持续时间大于1年）； 2.年龄18-80岁； 3.首次接受房颤导管消融术。

Inclusion criteria

1. Persistent atrial fibrillation (lasting more than 7 days but less than 1 year) or long-term persistent atrial fibrillation (lasting more than 1 year); 2. Age: 18 to 80 years old; 3. Received atrial fibrillation catheter ablation for the first time.

排除标准：

1.存在左心耳封堵或切除史； 2.术中未能成功转复窦性心律（包括电复律）； 3.严重的心脏瓣膜病需手术治疗； 4.甲状腺功能亢进未控制； 5.急性心肌梗死或不稳定型心绞痛； 6.妊娠期妇女； 7.无法提供知情同意； 8.既往有左心房消融或外科手术史。

Exclusion criteria：

1. History of left atrial appendage occlusion or surgical excision; 2. Failure to achieve successful conversion to sinus rhythm intraoperatively (including via electrical cardioversion); 3. Severe valvular heart disease requiring surgical intervention; 4. Uncontrolled hyperthyroidism; 5. Acute myocardial infarction or unstable angina pectoris; 6. Pregnancy; 7. Inability to provide informed consent; 8. History of previous left atrial ablation or cardiac surgery.
