研究目的：

对比自体血管基质组分（SVF）联合自固化磷酸钙人工骨与髂骨移植，和脂肪间充质干细胞（ADSC）联合自固化磷酸钙人工骨与髂骨移植治疗早中期股骨头坏死的临床效果，明确两种治疗方案在促进骨修复、改善髋关节功能及安全性方面的差异，为临床治疗方案的选择提供科学依据。  

Objectives of Study：

Compare the clinical effects of autologous vascular stromal fraction (SVF) combined with self-setting calcium phosphate artificial bone and iliac bone grafting, and adipose-derived mesenchymal stem cells (ADSC) combined with self-setting calcium phosphate artificial bone and iliac bone grafting in the treatment of early to mid-stage femoral head necrosis, clarifying the differences between the two treatment plans in promoting bone repair, improving hip joint function, and safety, providing a scientific basis for the selection of clinical treatment plans.

纳入标准：

纳入标准包括：年龄在 18 - 65 岁之间；自愿参与本研究并签署知情同意书

Inclusion criteria

Inclusion criteria include: age between 18 and 65 years; voluntary participation in this study and signing the informed consent form.

排除标准：

合并严重心脑血管疾病、血液系统疾病、感染性疾病；对手术及相关治疗有禁忌证；近期接受过其他影响股骨头坏死治疗的干预措施。

Exclusion criteria：

Combined with severe cardiovascular and cerebrovascular diseases, hematologic diseases, and infectious diseases; contraindications for surgery and related treatments; recently received other interventions affecting the treatment of femoral head necrosis.

1.手术过程中，先通过特定的分离技术从患者自体脂肪组织中提取SVF，随后将自固化磷酸钙人工骨与髂骨植入股骨头坏死区域，最后将提取的 SVF 注入植入物周围及坏死区域。

During the surgery, SVF is first extracted from the patient's autologous adipose tissue using specific isolation techniques, then self-setting calcium phosphate artificial bone and iliac bone are implanted into the necrotic area of the femoral head, and finally, the extracted SVF is injected around the implants and the necrotic area.

2.获取患者自体脂肪组织后，采用体外分离、培养、扩增技术获得 ADSC，在完成自固化磷酸钙人工骨与髂骨混合植入后，将培养好的 ADSC 悬液注射到股骨头坏死区域及植入物周围。两组手术操作均由同一组有经验的外科医生团队完成，确保手术操作的一致性。

2. After obtaining the patient's autologous adipose tissue, the ADSC is obtained using in vitro separation, culture, and amplification techniques. After completing the implantation of self-curing calcium phosphate artificial bone mixed with the iliac bone, the cultured ADSC suspension is injected into the area of femoral head necrosis and around the implant. Both surgical procedures are performed by the same experienced surgical team to ensure consistency in the surgical operation.
