1.构建手术前多因素AI预测模型:整合影像学(术前脑MRI检查)、术前检查、构建术前高效的AI预测模型,目标AUC≥0.90。模型可提前至手术前7天预测POCD,为临床干预留出窗口期。 2.临床价值与科学意义: 本研究通过预测心脏手术患者术后POCD的发生,手术前优化围手术期医护管理策略,从而降低术后POCD发生率,促进心脏患者术后快速康复。 3.社会经济效益:心脏术后POCD患者家庭护理成本增加40%,再入院率提升25%,患者家庭及社会医疗成本增加。该研究预计降低医疗费用15%-20%,同时节约医疗资源。 |

Objectives of Study: |
1. Construction of a multi-factor AI prediction model before surgery: Integrating imaging (preoperative brain MRI examination) and biomarkers (dynamic changes of IL-6, NSE, and GFAP before surgery), an efficient AI prediction model is constructed before surgery, with the goal of achieving an AUC of ≥0.90. The model can predict POCD up to 7 days before surgery, leaving a window period for clinical intervention. 2. Clinical value and scientific significance: This study predicts the occurrence of POCD in patients after cardiac surgery, optimizes perioperative medical and nursing management strategies before surgery, thereby reducing the incidence of POCD after surgery and promoting rapid recovery of cardiac patients after surgery. 3. Social and economic benefits: The family care cost for patients with POCD after cardiac surgery increases by 40%, and the readmission rate increases by 25%, increasing the medical costs for patients' families and society. This study is expected to reduce medical costs by 15% to 20% and save medical resources at the same time. |

1. 择期行冠状动脉搭桥术患者; 2. 术前蒙特利尔量表测试无认知障碍: (1) 文盲患者评分>=17 分; (2) 小学学历(教育年限<=6 年)患者评分>=20 分; (3) 初中及以上学历患者评分>=25 分; 3. ASA 评级为 II~IV 级; 4. 自愿签署手术、研究知情同意书; 5. 既往无心脏手术史; 6. 未参与其它临床研究; |

Inclusion criteria |
1. Patients scheduled for elective coronary artery bypass grafting (CABG); 2. No cognitive impairment on the Montreal Cognitive Assessment (MoCA) preoperatively: (1) Illiterate patients with score >= 17; (2) Patients with primary school education (education duration <= 6 years) with score >= 20; (3) Patients with junior high school or higher education with score >= 25; 3. ASA classification II–IV; 4. Voluntarily signed informed consent for surgery and study; 5. No previous history of cardiac surgery; 6. Not enrolled in any other clinical studies. |

排除标准: |
1. 中枢神经系统疾病或精神疾病病史者; 2. 长期服用抗抑郁药、镇静剂,或长期大量饮酒者(每周酒精摄入量>=350g); 3. 已参与其它临床试验或无法配合完成本研究; 4. 严重影响神经心理测评的视觉/听觉障碍; 5. 存在精神或神经疾病,术前、术后无法配合完成 POCD 评估; 6. 合并需同期手术的其他心血管疾病; 7. 有幽闭恐惧症; 8. 不能平卧; 9. 拒绝签署知情同意书; 10. 急诊心脏外科手术; 11. 既往脑外科手术史。 |

Exclusion criteria: |
1. Patients with a history of central nervous system or psychiatric disorders; 2. Those taking antidepressants or sedatives long-term, or long-term heavy drinkers (weekly alcohol intake >= 350g); 3. Those who have participated in other clinical trials or are unable to cooperate with this study; 4. Visual or auditory impairments severely affecting neuropsychological assessment; 5. Patients with mental or neurological disorders who cannot cooperate with POCD assessment before or after surgery; 6. Patients with other cardiovascular diseases requiring concurrent surgery; 7. Patients with claustrophobia; 8. Patients unable to lie flat; 9. Patients who refuse to sign the informed consent form; 10. Patients undergoing emergency cardiac surgery; 11. Patients with a history of neurosurgery. |
