研究目的：

本研究依托“基于健康档案的南京市区域卫生信息平台”和“南京市慢性病网络管理信息系统”，采用双向性队列研究方法，基于南京社区65岁及以上非心血管病人群的2018—2021年至少3次健康体检数据，应用群组化轨迹建模，建立腰围、体重指数、收缩压、舒张压、空腹血糖、血清总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇和甘油三酯等代谢危险因素随时间变化轨迹，构建单因素和多因素轨迹模型；并通过健康体检+医疗机构慢病报病，随访各亚组2022—2025年心血管病发病情况，比较各亚组心血管病的累计发病率，通过Cox比例风险回归模型分析不同亚组轨迹与新发心血管病的关联性，（1）科学精准确定各代谢危险因素最优轨迹模型；（2）系统描述老年人群和不同性别各代谢危险因素轨迹亚组人群的统计学分布特征；（3）全面评估不同代谢危险因素轨迹亚组心血管病发病风险；（4）积极探索老年人群的代谢因素轨迹水平与新发心血管病的关联性；（5）长期建立前瞻性随访的心血管病重点人群专病队列；（6）为社区老年人的心血管病风险评估提供多指标轨迹因素的流行病学研究提供数据支撑；（7）为政府制定65岁及以上人群心血管病的一级预防，合理分配卫生资源、优化资源配置提供决策依据。  

Objectives of Study：

This study employs a bidirectional cohort design based on the "Nanjing Regional Health Information Platform (Health Record-based)" and the "Nanjing Chronic Disease Network Management Information System," using data from community-dwelling adults aged ≥65 years without CVD who completed ≥3 check-ups (2018–2021). Group-based trajectory modeling will characterize longitudinal trajectories of metabolic risk factors (WC, BMI, SBP, DBP, FBG, TC, HDL-C, LDL-C, TG). Single- and multi-trajectory models will be constructed. Follow-up (2022–2025) combines health check-ups with chronic disease reporting to track incident CVD across subgroups. Cumulative incidence will be compared, and Cox models will analyze associations. Specific aims: (1) identify optimal trajectory models for each risk factor; (2) describe subgroup distribution overall and by sex; (3) evaluate CVD risk across subgroups; (4) explore associations between trajectories and incident CVD; (5) establish a prospective high-risk CVD cohort; (6) provide epidemiological support for multi-trajectory CVD risk assessment; (7) inform evidence-based prevention policies and resource allocation for adults >= 65 years.

纳入标准：

1.首次健康体检时年龄在65岁及以上； 2.2018—2021年间至少参加过3次健康体检，其中腰围、体重指数、收缩压、舒张压、空腹血糖、血清总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、甘油三酯等代谢性指标至少有3次均完整； 3.2018—2021年健康体检均未诊断为心血管病；

Inclusion criteria

1. Age 65 or above at the time of the initial health examination; 2. Completed at least three health examinations between 2018 and 2021, with at least three complete sets of metabolic indicators including waist circumference, body mass index, systolic blood pressure, diastolic blood pressure, fasting blood glucose, serum total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, and triglycerides; 3. No diagnosis of cardiovascular disease during any health examinations conducted between 2018 and 2021;

排除标准：

1.首次体检记录中社会人口学信息（性别、年龄、文化程度、家庭地址、联系方式等）、吸烟情况、饮酒情况、代谢指标记录缺失一项者； 2.2018—2021年间代谢性指标（BMI、收缩压、舒张压、空腹血糖和空腹血脂）均完整的体检记录不足3次； 3.入选队列时已被医疗机构确诊为心血管病或严重疾病晚 期患者或患有精神疾病；

Exclusion criteria：

1. Individuals with missing data in any one of the following categories within their initial health examination records: sociodemographic information (gender, age, educational attainment, residential address, contact details, etc.), smoking status, alcohol consumption, or metabolic indicators; 2. Individuals with fewer than three complete health examination records between 2018 and 2021 containing all metabolic indicators (BMI, systolic blood pressure, diastolic blood pressure, fasting blood glucose, and fasting blood lipids); 3. Individuals diagnosed by medical institutions with cardiovascular disease, advanced-stage severe illness, or mental disorders at the time of cohort inclusion;
