研究目的:

本项目依托一站式高清CT解剖成像与低剂量CT肺功能成像,系统评估DLIR对COPD患者吸呼双相CT影像定量指标的影响。以PFT为金标准,对比分析肺容积、气体潴留、肺密度变化等关键指标,筛选最优参数与成像方案,填补国内相关研究空白。 后续引入机器学习影像组学,建立相位差影像特征体系,融合结构与功能信息,构建高可解释性的AECOPD风险预测模型。 研究突破静态定量瓶颈,首次结合双相CT动态变化与影像组学特征,最终形成无创精准的COPD全周期管理工具,为个体化干预提供影像支持。  

Objectives of Study:

This project relies on one-stop high-definition CT anatomical imaging and low-dose CT pulmonary function imaging to systematically evaluate the impact of DLIR on quantitative indicators of inspiratory and expiratory phase CT images in COPD patients. Using PFT as the gold standard, it conducts comparative analysis of key indicators such as lung volume, air trapping, and lung density changes, screens the optimal parameters and imaging protocols, and fills the relevant domestic research gaps.Subsequently, machine learning radiomics methods are introduced to establish a phase difference imaging feature system, fuse structural and functional information, and construct a highly interpretable AECOPD risk prediction model.The study breaks through the bottleneck of static quantification, combines the dynamic changes of biphasic CT with radiomic features for the first time, and ultimately forms a non-invasive and accurate COPD full-cycle management tool, providing imaging support for individualized intervention.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄 ≥18 岁的成人COPD患者(诊断依据2025年GOLD指南);
2.同一次就诊中完成肺功能检查(PFT)及 GE Healthcare 256排 CT 双相(吸气/呼气)胸部平扫;
3.影像及临床资料完整,可进行配对分析。

Inclusion criteria

1. Adults aged >=18 years with a diagnosis of COPD, confirmed according to the 2025 GOLD guidelines. 2. Completion of both pulmonary function tests (PFT) and biphasic (inspiratory/expiratory) non-contrast chest CT scans on a GE Healthcare 256-slice CT scanner during the same clinical visit. 3. Availability of complete imaging and clinical data suitable for paired analysis.

排除标准:

1.图像存在明显运动伪影、严重金属伪影或其他影响定量分析与诊断质量的患者;
2.呼吸训练不能配合者;
3.合并其他严重肺部疾病(如肺纤维化、活动性肺结核)者。

Exclusion criteria:

1. Patients with images exhibiting significant motion artifacts, severe metal artifacts, or other artifacts that compromise the quality of quantitative analysis and diagnostic interpretation. 2. Patients unable to cooperate with breathing instructions during the CT scan. 3. Patients with concurrent severe pulmonary diseases, such as pulmonary fibrosis or active tuberculosis.
