研究目的:

当前研究存在空白:钙调磷酸酶抑制剂对口腔菌群衍生丁酸轴的调控机制、丁酸驱动牙龈成纤维细胞异常增殖的分子通路,以及口腔菌群-丁酸-免疫细胞-成纤维细胞的交互网络尚不明确。因此,本研究旨在通过优化临床队列设计明确钙调磷酸酶抑制剂使用者口腔菌群及代谢特征,利用唾液多组学整合分析解析口腔菌群与局部免疫环境的关联性及对牙龈成纤维细胞增殖的影响,借助“口腔菌群多组学时空映射技术(G-MOST)”绘制口腔菌群、代谢物及免疫细胞的交互网络并验证“口腔菌群代谢免疫轴(OMIA)”理论,最终揭示钙调磷酸酶抑制剂通过重塑口腔菌群及代谢物驱动牙龈成纤维细胞异常增殖的分子机制,为开发基于菌群及代谢物的无创早筛模型与干预策略提供依据。  

Objectives of Study:

There are gaps in current research: the regulatory mechanism of calcineurin inhibitors on oral flora derived butyrate axis, the molecular pathway of butyrate driving abnormal proliferation of gingival fibroblasts, and the interaction network of oral flora butyrate immune cells fibroblasts are still unclear. Therefore, this study aims to clarify the oral flora and metabolic characteristics of calcineurin inhibitor users by optimizing the clinical cohort design, analyze the correlation between oral flora and the local immune environment and its impact on gingival fibroblast proliferation by using saliva multi omics integrated analysis, draw the interactive network of oral flora, metabolites and immune cells by using "oral flora multi omics spatio-temporal mapping technology (g-most)" and verify the "oral flora metabolic immune axis (omia)" theory, and finally reveal the molecular mechanism of calcineurin inhibitor driving the abnormal proliferation of gingival fibroblasts by remodeling oral flora and metabolites, providing a basis for the development of non-invasive early screening model and intervention strategy based on flora and metabolites.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.肾移植术后伴发DIGO组(30例):肾移植术后时间≧1年,且入组前 3 个月内未接受牙周治疗或广谱抗生素治疗,肌酐值波动<10%。
2.移植术后无DIGO组(50例):肾移植术后时间≧1年,且入组前 3 个月内未接受牙周治疗或广谱抗生素治疗;其肌酐值波动<10%。
3.健康对照组(50例):无全身性疾病史,且待拔除的智齿术前一个月内未出现任何炎症表现,包括红肿或疼痛。

Inclusion criteria

1. Digo group after renal transplantation (30 cases): the time after renal transplantation was >= 1 year, and did not receive periodontal treatment or broad-spectrum antibiotic treatment within 3 months before enrollment, and the creatinine value fluctuated <10%. 2. No Digo group after transplantation (50 cases): the time after kidney transplantation was >= 1 year, and did not receive periodontal treatment or broad-spectrum antibiotic treatment within 3 months before enrollment; Its creatinine value fluctuated by <10%. 3. Healthy control group (50 cases): there was no history of systemic disease, and the wisdom tooth to be extracted did not appear any inflammatory manifestations, including swelling or pain, within one month before surgery.

排除标准:

1.长期抗生素使用患者;
2.有恶性肿瘤及免疫缺陷病史患者;
3.患有口腔黏膜疾病、癫痫或存在局部麻醉或全身手术禁忌症的患者。

Exclusion criteria:

1. Patients with long-term antibiotic use; 2. Patients with a history of malignant tumor and immunodeficiency; 3. Patients with oral mucosal disease, epilepsy or contraindications to local anesthesia or general surgery.

电子化数据采集:使用REDCap(Research Electronic Data Capture)系统进行临床数据的实时录入,包括患者基本信息(年龄、性别、BMI)、手术记录、样本采集时间等,确保数据格式标准化(如日期格式YYYY-MM-DD,BMI保留至小数点后1位)。实验室数据(宏基因组、代谢组、单细胞数据)通过LIMS(实验室信息管理系统)自动导入,避免手动转录错误。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Electronic data acquisition: redcap (research electronic data capture) system is used for real-time input of clinical data, including basic information of patients (age, sex, BMI), operation records, sample collection time, etc., to ensure the standardization of data format (such as date format yyyy-mm-dd, BMI is retained to 1 decimal place). Laboratory data (metagenome, metabolome, single-cell data) are automatically imported through LIMS (laboratory information management system) to avoid manual transcription errors.
