研究目的：

通过给予不同剂量的盐酸右美托咪定鼻喷雾剂，观察其对椎管内麻醉穿刺操作的镇静、遗忘程度，为临床上安全有效地使用盐酸右美托咪定鼻喷雾剂提供依据。  

Objectives of Study：

By administering different doses of dexmedetomidine hydrochloride nasal spray, the study observes its effects on sedation and amnesia during spinal anesthesia puncture procedures, providing a basis for the safe and effective clinical use of dexmedetomidine hydrochloride nasal spray.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 签署知情同意书； (2) 本院择期行骨科下肢手术的患者； (3) 年龄18-65岁； (4) ASA分级（美国麻醉医师协会身体状况分级）为 I 或 II 级。

Inclusion criteria

1. Sign the informed consent form; 2. Patients scheduled for elective orthopedic lower limb surgery at this hospital; 3. Aged 18-65 years; 4. ASA classification (American Society of Anesthesiologists physical status) of I or II.

排除标准：

(1) 长期或近期服用抗精神病、镇静或镇痛药物的病史； (2) 严重心、肺、肝、肾功能不全（ASA III级或以上）； (3) 腰椎疾患和腰痛病史； (4) 凝血功能异常； (5) 正在参与另一项临床试验。

Exclusion criteria：

1. History of long-term or recent use of antipsychotic, sedative, or analgesic drugs; 2. Severe heart, lung, liver, or kidney dysfunction (ASA grade III or above); 3. History of lumbar spine disorders and low back pain; 4. Abnormal coagulation function; 5. Currently participating in another clinical trial.

纳入研究的患者按1:1:1的比例被分配到25ug盐酸右美托咪定鼻喷雾剂(D1)组，50ug盐酸右美托咪定鼻喷雾剂(D2)组，0.9%氯化钠溶液（NS）组中。计算机生成的随机化方案由一名非盲法统计学家制定，分配方案装在按顺序编号的密封不透明信封中，不对研究人员公开。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Patients included in the study were randomly assigned in a 1:1:1 ratio to the 25 μg dexmedetomidine hydrochloride nasal spray (D1) group, the 50 μg dexmedetomidine hydrochloride nasal spray (D2) group, or the 0.9% sodium chloride solution (NS) group. The computer-generated randomization scheme was developed by an unblinded statistician, and the allocation was placed in sequentially numbered, sealed opaque envelopes, which were not disclosed to the researchers.

对患者在资格评估和正式试验登记后立即打开随机化信封。由于患者需要遵守方案，因此不对其进行盲法操作。但是，外科医生、麻醉师、统计人员以及负责评估神经认知功能和术后临床结果的研究小组成员对患者的分配和治疗并不知情

Blinding：

For patients, the randomization envelopes are opened immediately after eligibility assessment and formal trial registration. Since patients need to adhere to the protocol, they are not blinded. However, surgeons, anesthesiologists, statisticians, and research team members responsible for assessing neurocognitive function and postoperative clinical outcomes are unaware of the patients' allocation and treatment.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

收 141 例患者的基础信息、AS评分、 血液动力学信息、术后睡眠质量、VAS评分、是否发生呕吐等，由专人用 CRF、电子病例等记录，设盲法和人员培训控质。数据管理：Excel 双人录入，SPSS 分析，双人核对，定期审查。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Collected basic information, AS scores, hemodynamic data, postoperative sleep quality, VAS scores, and occurrence of vomiting in 141 patients, which were recorded by dedicated personnel using CRF and electronic medical records, with blinding and staff training to control quality. Data management: dual entry in Excel, analysis with SPSS, double verification, and regular review.
