研究目的:

本研究旨在系统阐明运动干预对青少年儿童生长发育的促进作用及其内在生理机制。研究首先通过分析单次标准化运动后生长激素分泌动力学参数(包括峰值浓度、曲线下面积及峰值停留时间)与骨代谢标志物的动态变化,揭示急性运动的生理应答特征并建立运动强度与生理反应之间的量效关系。  

Objectives of Study:

The purpose of this study is to systematically clarify the role of exercise intervention in promoting the growth and development of young children and its internal physiological mechanism. Firstly, by analyzing the dynamic changes of growth hormone secretion kinetic parameters ( including peak concentration, area under the curve and peak residence time ) and bone metabolism markers after a single standardized exercise, the physiological response characteristics of acute exercise were revealed and the dose-effect relationship between exercise intensity and physiological response was established.

纳入标准:

1.年龄6-16岁 2.家长有意愿改善儿童身高 3.骨龄要求:经左手腕部X光片+Greulich-Pyle骨龄评估法,骨龄<=14岁(男性)、骨龄<=13岁(女性,因女性骨骺闭合早,单独明确上限),且骨龄<=实际年龄+1岁,确保骨骺未闭合、存在身高增长空间;? 4.身高水平:符合《中国7岁以下儿童生长发育参照标准》(6-9岁适用)或《中国青少年生长发育参照标准》(10-16岁适用),身高位于同年龄、同性别P10~P90百分位 5.营养状况:无严重偏食、厌食;由家长协助完成3天饮食日记,确认每日蛋白质(奶、蛋、肉、豆制品)摄入量>=同年龄推荐值的70%(6-9岁可放宽至70%,10-16岁建议≥80%)。? 6.睡眠情况:每日睡眠时间>=8-9小时 7.无不良习惯:电子产品使用<=1.5-2小时/天(6-9岁<=1.5小时,10-16岁<=2小时),无吸烟、饮酒、熬夜(10岁及以上重点排查吸烟饮酒,6-9岁重点排查熬夜)。

Inclusion criteria

1. Age 6-16 years old 2.Parents are willing to improve the child's height. 3. Bone age requirement: Bone age <=14 years old (male), bone age <=13 years old (female, because female epiphysis closes early, separately specify the upper limit), and bone age <=actual age + 1 year old, ensure that the epiphysis is not closed and there is room for height growth, according to the X-ray film of the left wrist and the Greulich-Pyle Bone Age Evaluation Method. 4. Height level: in accordance with the Chinese Reference Standards for the Growth and Development of Children Under 7 Years of Age (for 6-9 year olds) or the Chinese Reference Standards for the Growth and Development of Adolescents (for 10-16 year olds), with heights in the P10~P90 percentile for the same age and gender. 5. Nutritional status: no serious deviation from food or anorexia; parents' assistance in completing a 3-day dietary diary, confirming that the daily protein intake (milk, egg, meat, soybean products) is >=70% of the recommended value for the same age (6-9 years old can be relaxed to 70%, 10-16 years old ≥80%). 6. Sleep: daily sleep time >= 8-9 hours 7. No bad habits: use of electronic products <=1.5-2 hours/day (6-9 years old <=1.5 hours, 10-16 years old <=2 hours), no smoking, drinking, staying up late (10 years old and above focus on smoking and drinking, 6-9 years old focus on staying up late).

排除标准:

1.患有已知会严重影响生长发育或骨骼代谢的疾病,如生长激素缺乏症、甲状腺功能异常、性早熟、慢性肾病、先天性骨骼疾病或糖尿病等; 2. 近6个月内曾有上肢或下肢的长骨骨折史,或存在影响运动能力的骨科疾病(如严重的关节炎、韧带损伤); 3.长期(超过1个月)服用可能干扰生长激素分泌或骨代谢的药物,如糖皮质激素、性激素、哌甲酯等; 4.存在心肺功能不全、严重心律失常等运动禁忌症,或不适合参与运动试验的其他健康状况; 5.已被诊断为注意力缺陷多动障碍、自闭症谱系障碍等可能影响其理解和配合完成标准化运动干预的神经发育障碍。

Exclusion criteria:

1. suffering from diseases known to seriously affect growth and development or bone metabolism, such as growth hormone deficiency, thyroid dysfunction, precocious puberty, chronic kidney disease, congenital bone disease or diabetes ; 2. Have a history of long bone fractures of the upper or lower limbs in the past 6 months, or have orthopedic diseases that affect exercise capacity ( such as severe arthritis, ligament injury ) ; 3.Long-term ( more than 1 month ) administration of drugs that may interfere with growth hormone secretion or bone metabolism, such as glucocorticoids, sex hormones, methylphenidate, etc. ; 4.Exercise contraindications such as cardiopulmonary insufficiency and severe arrhythmia, or other health conditions that are not suitable for participating in exercise tests ; 5.Neurodevelopmental disorders that have been diagnosed as attention deficit hyperactivity disorder and autism spectrum disorder may affect their understanding and cooperation to complete standardized exercise intervention.

准备: 受试者禁食过夜,于清晨安静状态下采集基线(T0)静脉血。 运动方案: 随后进行单次标准化运动,总时长约40-60分钟。方案严格遵循“热身(约15-20分钟)-主体训练(约30-40分钟)-整理活动(约10-15分钟)”的结构。主体训练以多维跳跃动作为核心(如开合跳、跳箱、垂直纵跳等),运动强度通过实时心率和主观疲劳感觉进行个体化控制,目标强度为中高强度(HRR 70-85%)。 采样: 于运动后即刻(T1)、30分钟(T2)及60分钟(T3)分别采集系列静脉血。

Intervention:

Preparation : The subjects were fasted overnight, and baseline ( T0 ) venous blood was collected in the quiet state in the morning. Exercise regimen : Subsequently, a single standardized exercise was performed for a total duration of about 40-60 minutes. The program strictly follows the structure of ' warm-up ( about 15-20 minutes ) -subject training ( about 30-40 minutes ) -organizing activities ( about 10-15 minutes ) '. The main body training takes multi-dimensional jumping movements as the core ( such as open and close jumps, jump boxes, vertical vertical jumps, etc. ). The exercise intensity is individually controlled by real-time heart rate and subjective fatigue sensation, and the target intensity is medium and high intensity ( HRR 70-85 % ). Sampling : A series of venous blood was collected immediately after exercise ( T1 ), 30 minutes ( T2 ) and 60 minutes ( T3 ).
