研究目的：

1. 评估术前链霉蛋白酶服用后坐位按摩相较于单纯坐位活动，在改善胃镜检查过程中胃内黏液残留、气泡干扰及胃黏膜显露清晰度方面的临床效果； 2. 明确术前链霉蛋白酶服用后联合坐位按摩的干预方案对胃镜操作时间、视野干扰程度及患者依从性等关键指标的影响； 3. 为优化胃镜术前准备流程、提升胃镜检查质量与操作效率提供高质量的循证医学依据。  

Objectives of Study：

1. Evaluate whether seated massage following preoperative streptokinase administration is superior to seated activity in improving gastric mucus residue, bubble interference, and gastric mucosal visualization clarity during gastroscopy; 2. Determine the effects of combined seated massage and streptokinase administration on gastroscopy procedure duration, visual field interference, and patient compliance; 3. Provide evidence-based guidance for optimizing preoperative gastroscopy preparation protocols and enhancing examination quality and efficiency.

纳入标准：

1. 能够配合胃镜手术； 2. 内窥镜检查前停用抗凝药物1周； 3. 自愿参与并提供书面知情同意书。

Inclusion criteria

1. Able to undergo gastroscopy; 2. Discontinue anticoagulant medications one week prior to endoscopy; 3. Voluntarily participate and provide written informed consent.

排除标准：

1. EGD禁忌症； 2. 怀孕或哺乳； 3. 对链霉蛋白酶或西甲硅油过敏； 4. 心肺肾等重要脏器功能不全、不能耐受胃镜及麻醉者； 5. 肝硬化伴食管胃底静脉曲张； 6. 活动性消化道出血； 7. 胃肠道手术史； 8. 严重的胃潴留； 9. 不能配合完成坐位活动法和按摩法的患者。

Exclusion criteria：

1. Contraindications of EGD; 2. Pregnancy or lactation; 3. Allergic to streptokinase or simethicone; 4. Patients with impaired function of vital organs such as the heart, lungs, and kidneys, or those who cannot tolerate endoscopy and anesthesia; 5. Liver cirrhosis with esophageal and gastric varices; 6. Active gastrointestinal bleeding; 7. History of gastrointestinal surgery; 8. Severe gastric retention; 9. Patients who are unable to cooperate with seated exercises and massage techniques.

Thirty to sixty minutes prior to gastroscopy, administer the streptokinase solution orally. Instruct the patient to sit in the waiting area chair, grasp both armrests, and perform the following movements: recline backward, lean forward, tilt left, and tilt right. Repeat this sequence five times to ensure the medication fully contacts the gastric mucosa.

于胃镜检查前30-60 min口服链霉蛋白酶溶液后嘱患者在等候区座椅上，取高坐位或半坐位（靠背约倾斜45~60°），背部有支撑，放松肩膀，双手自然置于大腿，尽量保持呼吸平稳，以上腹部（剑突下至脐上区域）为核心区域，即胃大致所在部位，按摩范围包括左上腹（胃体区）、中上腹（胃窦区）、右上腹（幽门区），研究人员指导患者使用手掌根部或四指并拢，在上腹部进行顺时针环形推揉，力度适中，按摩时间以3–5分钟为宜；同时，用手掌轻轻叩击上腹部各象限（尤其胃体区域），每区10秒，促进气泡破裂与内容物均匀分布。保持按摩节奏舒缓、手法柔和，避免引起疼痛或恶心。

Intervention：

After taking the streptokinase solution orally 30-60 minutes prior to gastroscopy, instruct the patient to sit in the waiting area chair in an upright or semi-upright position (backrest tilted approximately 45-60°), with back support. Relax the shoulders, place both hands naturally on the thighs, and maintain steady breathing as much as possible. Focus on the upper abdomen (area from below the xiphoid process to above the umbilicus), corresponding to the approximate location of the stomach. The massage area includes the left upper abdomen (body of the stomach), middle upper abdomen (antrum), and right upper abdomen (pylorus). Researchers instruct patients to use the heel of the palm or four fingers together to perform clockwise circular kneading motions on the upper abdomen with moderate pressure. The massage duration should be 3–5 minutes. Simultaneously, gently tap each quadrant of the upper abdomen (particularly the gastric body area) with the palm for 10 seconds per section to promote bubble rupture and even distribution of contents. Maintain a gentle rhythm and soft technique throughout the massage to avoid causing pain or nausea.
