研究目的：

本课题以社区的居家老人为研究对象，对确诊吞咽障碍老年患者应用医院-社区-家庭三元联动康复干预，探索促进社区居家老年患者早期康复的有效措施，以实现老年人吞咽障碍早发现早预防早干预。  

Objectives of Study：

This study focuses on elderly people living at home in the community. It applies a tertiary - linkage rehabilitation intervention involving the hospital, community, and family for elderly patients diagnosed with dysphagia. The aim is to explore effective measures to promote early rehabilitation of elderly patients living at home in the community, so as to achieve early detection, early prevention, and early intervention of dysphagia in the elderly.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=60岁； 2.已在社区卫生服务中心建立管理健康档案的居家老人； 3.认知功能正常，神志清楚，能理解、执行简单指令及正确回答问题； 4.知情同意，愿意参加本研究者。

Inclusion criteria

1. Age >= 60 years; 2. Elderly people who have established health management records at the community health service center; 3. Normal cognitive function, clear consciousness, able to understand and follow simple instructions and answer questions correctly; 4. Informed consent, willing to participate in this study.

排除标准：

1.合并心、肝、肾等严重身体疾病或残疾，生活不能自理； 2.中重度认知障碍、痴呆或精神疾病； 3.正在参与其他干预研究者； 4.中途退出本次研究者。

Exclusion criteria：

1. Severe physical diseases or disabilities involving the heart, liver, kidneys, etc., resulting in inability to care for oneself; 2. Moderate to severe cognitive impairment, dementia, or mental illness; 3. Individuals currently participating in other intervention studies; 4. Individuals who withdraw from this study midway.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究采用纸质病例报告表（CRF）联合电子数据采集系统（EDC）进行数据管理。所有数据由经过培训的研究人员根据原始记录和实验室报告录入，确保及时、准确、完整。数据管理员每月进行质量审核。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data are collected using paper Case Report Forms (CRFs) and an Electronic Data Capture (EDC) system. All entries are completed by trained research staff based on original records and laboratory reports. Data are reviewed monthly for quality assurance.
