研究目的：

1. 问题： （1）我国人口老龄化日益加剧，合并肌少症老年患者更容易发生跌倒并引起髋 部骨折，行手术治疗能降低髋关节成为“人生的最后一次骨折”，老年肌少症患 者使用肌松药后更以发生肌力减弱，患者肌力弱增加了术后呼吸相关不良事件的 发生率，增加了死亡率。 （2）传统的肌松拮抗药物-新斯的明/格隆溴铵通过抑制胆碱酯酶减少乙酰胆 碱的分解间接发挥作用，对深肌松的拮抗作用不明显，具有封顶效应，且易引起 分泌物增多、气道阻力增加及心律失常等，对老年肌少症患者的快速康复改善不 明显。 2. 意义 （1）舒更葡萄钠具有特定的化学结构，可以快速有效地逆转罗库溴铵或维库 溴铵诱导的不同程度的神经肌肉阻滞，且不良反应更少，对于老年肌少症患者 全麻后的肌松改善理论上具有较大优势，对于这类高危患者的围术期安全提供 了一种新的保障。 （2）舒更葡糖钠国产后，价格较为亲民且纳入了医保，对于其在特殊患者中应用的探索有了一定意义。  

Objectives of Study：

1. Issue: (1) China's population is ageing at an increasingly rapid pace. Elderly patients with sarcopenia are more prone to falls leading to hip fractures. Surgical intervention can prevent the hip from becoming the "final fracture of life". However, sarcopenic elderly patients experience further muscle weakness following neuromuscular blocking agents. This diminished muscle strength increases the incidence of respiratory-related adverse events post-surgery, thereby raising mortality rates. (2) Traditional neuromuscular blocking agents—neostigmine/ Glycopyrronium works indirectly by inhibiting acetylcholinesterase to reduce acetylcholine breakdown. It exhibits limited antagonistic effect on deep muscle relaxation, demonstrates a ceiling effect, and commonly induces increased secretions, heightened airway resistance, and arrhythmias. Its contribution to enhancing rapid recovery in elderly patients with sarcopenia is negligible. 2. Significance (1) Sodium sugammadex possesses a specific chemical structure enabling rapid and effective reversal of varying degrees of neuromuscular blockade induced by rocuronium or vecuronium, with fewer adverse reactions. Theoretically, it offers significant advantages for improving neuromuscular blockade in elderly patients with sarcopenia following general anaesthesia, providing a new safeguard for perioperative safety in this high-risk population. (2) Following its domestic production, suxamethonium gluconate sodium has become more affordable and is now covered by medical insurance, making its exploration for use in special patient groups particularly meaningful.

纳入标准：

1.摔伤，单侧髋部骨折，接受人工股骨头置换术（artificial femoral head replacement，AFHR）或股骨近端防旋髓内钉( proximal femoral nail antirotation，PFNA) 手术患者； 2.术前行胸部CT检查第12胸椎水平骨骼肌肌肉质量指数(SMI),男性:SMI＜42.6cm^2/m^2、女性: SMI＜30.6cm^2/m^2； 3.麻醉诱导和维持过程肌松药使用罗库溴铵或维库溴铵。

Inclusion criteria

1.Patients with unilateral hip fracture caused by falling injury, who underwent artificial femoral head replacement (AFHR) or proximal femoral nail antirotation (PFNA) surgery 2.Preoperative chest CT examination was performed to measure the skeletal muscle mass index (SMI) at the level of the 12th thoracic vertebra. For males: SMI < 42.6cm^2/m^2; For females: SMI < 30.6cm^2/m^2 3.Rocuronium or vecuronium was used as muscle relaxants during anesthesia induction and maintenance

排除标准：

1.伤前存在下肢运动功能障碍，合并冠心病、脑梗死和脑卒中等基础疾病史; 2.非首次髋部骨折，交通事故，多发骨折，先天性或后天性髋关节发育异常，病理性骨折; 3.非AFHR或PFNA术式; 4.保守治疗; 5.诱导选择了顺势阿曲库铵; 6.严重肝肾功能不全; 7.相关药物过敏等。

Exclusion criteria：

1. Pre-existing lower limb motor dysfunction, with a history of underlying diseases such as coronary heartdisease, cerebral infarction and stroke 2. Non-first-time hip fracture, traffic accident, multiple fractures, congenital or acquired hip dysplasia,pathological fracture 3.Non-AFHR Or PFNA surgical procedure 4. Conservative treatment 5.Cisatracurium besilate was selected for induction 6. Severe hepatic and renal insufficiency 7. Allergic to related drugs, etc.

One hour before surgery, Microsoft Office 365 Excel (Microsoft, Redmond, WA, http://www.microsoft.com) was used to randomly divide the participants into the sugammadex group (Group S, n=50) and the neostigmine group (Group N, n=50). The specific method was as follows: the researchers arranged the patients' hospital numbers and study groups into two columns. In the "Group" column, the "RAND" function was used to generate random numbers 1 or 2, where 1 represented Group S and 2 represented Group N.
