研究目的:

以血培养结果为金标准,评估环介导等温扩增联合微流控芯片技术在疑似导管相关血流感染患者中检测金黄色葡萄球菌、表皮葡萄球菌、屎肠球菌、粪肠球菌、大肠杆菌、肺炎克雷伯杆菌、铜绿假单胞菌、鲍曼不动杆菌、阴沟肠杆菌等常见病原菌的诊断效能,探索该检测技术在快速诊断导管相关性血流感染的价值。避免临床中非必要导管拔除,减轻医护人员工作和患者经济负担。  

Objectives of Study:

Using the blood culture results as the gold standard, the diagnostic efficacy of the loop-mediated isothermal amplification combined with microfluidic chip technology in detecting common pathogenic bacteria such as Staphylococcus aureus, Staphylococcus epidermidis, Enterococcus faecalis, Enterococcus faecium, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter baumannii, and Enterobacter cloacae in patients with suspected catheter-related bloodstream infections was evaluated. This study aimed to explore the value of this detection technology in the rapid diagnosis of catheter-related bloodstream infections. It also aimed to avoid unnecessary catheter removal in clinical practice, thereby reducing the workload of medical staff and the economic burden on patients.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄≥18周岁; 2.留置中心静脉导管(如锁骨下静脉导管、颈内静脉导管、股静脉导管等); 3.存在发热(体温≥38.0℃)或低温(体温≤36.0℃)、寒战、低血压(收缩压< 90mmHg)等疑似血流感染的临床表现且医生开具血培养检测医嘱的患者; 4.自愿参加本研究并签署知情同意书。

Inclusion criteria

1. Age >= 18 years old; 2. Central venous catheter (such as subclavian vein catheter, internal jugular vein catheter, femoral vein catheter, etc.) is in place; 3. Present with suspected bloodstream infection clinical manifestations such as fever (body temperature >= 38.0℃) or hypothermia (body temperature <= 36.0℃), chills, hypotension (systolic blood pressure < 90mmHg), etc., and the doctor has prescribed blood culture test instructions; 4. Voluntarily participate in this study and sign the informed consent form.

排除标准:

1.近3天内已接受过针对血流感染的足量、有效抗菌药物治疗,且临床症状明显改善; 2.存在严重凝血功能障碍(如血小板计数<50×10?/L、凝血酶原时间国际标准化比值>2.0等),无法进行血液标本采集; 3.患有精神疾病或认知障碍,无法配合研究相关操作及随访; 4.妊娠或哺乳期妇女。

Exclusion criteria:

1. Within the past 3 days, adequate and effective antibacterial drugs have been administered for the treatment of bloodstream infection, and the clinical symptoms have significantly improved; 2. There is severe coagulation dysfunction (such as platelet count < 50×10^9/L, international normalized ratio of prothrombin time > 2.0, etc.), making blood sample collection impossible; 3. Suffering from mental illness or cognitive impairment, unable to cooperate with the relevant research operations and follow-up; 4. Pregnant or lactating women.
