研究目的：

本研究旨在聚焦痴呆老人及其家庭照顾者居家到养老机构照护的过渡前决策阶段，明确痴呆家庭过渡决策时机，识别影响其决策过程的因素及支持需求；在此基础上，形成干预要素并针对不同痴呆严重程度的老人机器家庭照顾者的需求构建过渡前决策支持方案，初步验证该方案的可行性与有效性，为优化痴呆照护过渡提供理论依据与实践参考。  

Objectives of Study：

This study aims to focus on the pre-transition decision-making phase during the shift from home care to institutional care among older adults with dementia and their family caregivers. It seeks to determine the appropriate timing for transition-related decisions, identify factors influencing the decision-making process, and explore the support needs of dementia families. Based on these findings, the study will develop core intervention components and construct a pre-transition decision support program tailored to the needs of older adults with varying levels of dementia severity and their family caregivers. The feasibility and preliminary effectiveness of the program will be evaluated to provide theoretical grounding and practical guidance for optimizing care transitions in dementia.

纳入标准：

痴呆老人：①年龄≥60岁；②患有痴呆；③自愿参加本次研究。 家庭照顾者：①主要家庭照顾者（≥18岁，照护时间≥6个月）；②照顾者自愿参与。

Inclusion criteria

People with dementia: 1.Aged >= 60 years;2. Diagnosed with dementia;3.Willing to participate in the study. Family caregivers: 1. Primary family caregiver (aged >= 18 years and providing care for >= 6 months);2. Willing to participate in the study.

排除标准：

痴呆老人：①无法配合干预或意识障碍或急性精神行为异常；②终末期疾病者。 家庭照顾者：家庭已完全进入机构照护。

Exclusion criteria：

People with dementia: 1.Unable to cooperate with the intervention, or presenting with impaired consciousness or acute psychiatric/behavioral disturbances; 2. Diagnosed with a terminal illness. Family caregivers: The family has already fully transitioned to institutional care.
