研究目的：

通过回顾性分析我科室自2015年至今近端胃癌实施手术的患者，比较胃上接受PG和TG后各组患者淋巴结清扫数目、近期及远期并发症、总体生存率、无病生存率等指标，探讨 PG的安全性及预后，并为胃上部癌手术方式的选择提供依据。  

Objectives of Study：

Through a retrospective analysis of patients with proximal gastric cancer who underwent surgery in our department since 2015, we compared the number of lymph nodes dissected, short- and long-term complications, overall survival, and disease-free survival between the PG and TG groups. This study aims to evaluate the safety and prognosis of PG and to provide evidence for the selection of surgical approaches in proximal gastric cancer.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）2015年至今我科室收治的进展期胃上部癌，包括SiewertⅡ型和Ⅲ型食管胃结合部腺癌、胃底癌和胃体上段癌患者； （2）术后病理回报为腺癌； （3）临床资料完整； （4）手术方式为全胃切除术或近端胃切除术。

Inclusion criteria

(1) Patients with advanced proximal gastric cancer treated in our department since 2015, including Siewert type II and III adenocarcinoma of the esophagogastric junction, gastric fundus cancer, and upper gastric body cancer; (2) Postoperative pathological diagnosis confirmed adenocarcinoma; (3) Complete clinical data available; (4) Surgical procedures included total gastrectomy or proximal gastrectomy.

排除标准：

（1）合并其他部位恶性肿瘤； （2）临床资料缺失； （3）存在严重的心肺功能障碍、肝肾功能不全等手术禁忌证； （4）失访。

Exclusion criteria：

(1) Patients with concurrent malignancies in other sites; (2) Incomplete clinical data; (3) Presence of severe cardiopulmonary dysfunction, hepatic or renal insufficiency, or other contraindications to surgery; (4) Loss to follow-up.

数据采集和管理由两部分组成：病例记录表（Case Record Form, CRF）和本地维护的数据库。患者信息、围手术期变量及随访数据首先记录在 CRF 上，然后录入 Microsoft Excel 电子表格进行集中管理。Excel 文件存储在有密码保护的院内计算机中，仅限授权研究人员访问。所有数据均由两名独立研究人员进行双人核对，以确保准确性和完整性。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection and management consist of two components: the Case Record Form (CRF) and a locally maintained database. Patient information, perioperative variables, and follow-up data are first recorded on the CRF and then entered into a Microsoft Excel spreadsheet for centralized management. The Excel file is stored on a password-protected institutional computer, with access restricted to authorized investigators. Data entry is double-checked by two independent researchers to ensure accuracy and completeness.
