探讨茵陈蒿汤对危重患者胆汁淤积症及整体预后的影响。  

Objectives of Study：

To investigate the effect of Yinchenhao Decoction on cholestasis and prognosis of critically ill patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

于沧州市中心医院急诊医学部ICU住院的接受机械通气的重症患者：1.年龄>=18岁；2.急性生理与慢性健康-Ⅱ（APACHE-Ⅱ）评分＞8分。

Inclusion criteria

The critically ill patients receiving mechanical ventilation admitted to ICU of Emergency Department of Cangzhou Central Hospital: 1. Age>=18 years old; 2. Acute Physiology and Chronic Health Evaluation-Ⅱ(APACHE-Ⅱ) scores > 8 points.

排除标准：

年龄<18岁、住院时间＜24h、孕妇、既往有胆囊结石、胆管结石、肝内外胆管疾病、肝硬化病史者、存在急性胆囊炎、胆管炎者、存在机械性肠梗阻者、因消化道手术等原因预期7天内无法进行肠内营养者、因腹部胀气等原因无法进行超声相关数据测定者、癌症晚期者、多脏器功能衰竭终末期患者、未获得知情同意者。

Exclusion criteria：

Age <18 years old; length of hospitalization < 24h; pregnany; patients with previous history of gallstones, bile duct stones, intrahepatic and intrahepatic bile duct diseases, cirrhosis; patients with acute cholecystitis, cholangitis, or mechanical intestinal obstruction; patients who could not receive enteral nutrition within 7 days due to gastrointestinal surgery or other reasons; patients who could not receive ultrasonic examination due to abdominal distension or other reasons; and patients with advanced cancer, end-stage multiple organ failure or without informed consent.

茵陈蒿汤治疗组：茵陈蒿汤治疗＋常规治疗。茵陈蒿汤治疗方案：中药治疗＋常规治疗。中药治疗方案：鼻饲茵陈蒿汤（茵陈15g，栀子10g，大黄5g），2次/日，早晚各一次，7天为一疗程。

Yinchenhao Decoction Treatment Group: Yinchenhao Decoction treatment + conventional treatment. Yinchenhao Decoction treatment plan: Herbal medicine treatment + conventional treatment. Herbal medicine treatment plan: Nasogastric administration of Yinchenhao Decoction (composed of Artemisia capillaris 15g, Gardenia jasminoides 10g, and Rheum palmatum 5g), twice daily (morning and evening), with a treatment course of 7 days.
