研究目的：

（ 1 ）基于同期网络方法构建老年 HF 患者社会衰弱身心症状的网络结构，分析 同期网络中身心症状的桥梁症状和核心症状。 （2 ）基于交叉滞后网络方法分析不同时间点身心症状网络随时间变化的因果 关系。  

Objectives of Study：

(1) Based on the concurrent network method, construct the network structure of physical and mental symptoms of social frailty in elderly patients with HF, and analyze the bridge symptoms and core symptoms of physical and mental symptoms in the concurrent network. (2) Analyze the causal relationship of the physical and mental symptom network at different time points over time based on the cross-lag network method.

纳入标准：

（ 1 ）符合《中国心力衰竭诊断和治疗指南 2024》诊断标准，临床确诊为 HF 患 者； （2 ）社会衰弱性指数（SVI ）评分≥12.25 分； （3）纽约心功能分级为 II-IV 级； （4 ）年龄≥60 岁； （5 ）能够进行 日常的语言沟通和交流， 自愿参加本研究。

Inclusion criteria

1. patients who met the diagnostic criteria of "Chinese Guidelines for the Diagnosis and Treatment of Heart Failure 2024" and were clinically diagnosed with HF; 2. Social frailty index (SVI) score >=12.25; 3. New York Heart function class II-IV; 4. age >=60 years old; 5. Able to communicate in daily language and volunteered to participate in this study.

排除标准：

1 .心脏移植术后； 2.合并严重认知功能障碍（MMSE≤9 分）、严重精神疾病、严重肝 肾功能不全、恶性肿瘤； 3.正在参与其他临床研究。

Exclusion criteria：

1. After heart transplantation; 2. Combined with severe cognitive impairment (MMSE<=9), severe mental illness, severe liver and kidney dysfunction, malignant tumor; 3. Participating in other clinical studies.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

采用问卷调查法进行资料收集。调查问卷由研究者在入院48h内亲自发放，并向研究对象说明调查的目的、意义和方法，获得研究对象知情同意后签署知情同意书。尽量使研究对象在不受干扰的状况下自行填写。若因视力、文化程度等原因不能自行填写者，可由监护人、代理人或研究者逐条询问后代为填写。问卷发放过程中研究对象若有疑问，研究者应及时给予解答，问卷发放完毕后，当场回收并检查核对；如有漏项，及时请研究对象补充。在患者出院后3个月、6个月时由研究者本人进行门诊或电话随访。根据患者的具体情况，选择患者方便的时间进行社会衰弱数据的收集，若患者无法接听电话则由患者的照顾者根据患者情况回答。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

In this study, questionnaire survey was used for data collection. The questionnaire was distributed by the researcher within 48 hours after admission, and the purpose, significance and methods of the survey were explained to the research object. The informed consent was obtained from the research object and the informed consent was signed. As far as possible, the subjects were allowed to fill in the information by themselves without interference. If they cannot fill in the form by themselves due to vision, education level and other reasons, their guardians, agents or researchers can ask their offspring to fill in the form one by one. If the subjects had any questions during the distribution of the questionnaire, the researchers should provide timely answers. After the questionnaire was distributed, the questionnaire was collected and checked on the spot. If there are any missing items, please ask the research subjects to supplement in time. Patients were followed up at 3 months and 6 months after discharge by the investigators themselves in clinic or by telephone. According to the specific conditions of the patients, the convenient time of the patients was selected for the collection of social frailty data. If the patients were unable to answer the phone, the caregivers of the patients answered according to the patients' conditions.
