研究目的:

本研究的目的是借助人工智能技术建立基于眼底彩照的CSVD风险预测模型,辅助大规模人群CSVD的早期筛查,改善脑血管疾病病人的远期预后,减轻家庭和社会相关经济负担。  

Objectives of Study:

The purpose of this study is to establish a CSVD risk prediction model based on fundus color photographs using artificial intelligence technology, assist in the early screening of CSVD in large-scale populations, improve the long-term prognosis of patients with cerebrovascular diseases, and reduce the relevant economic burden on families and society.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄>=18岁; 2.就诊于北京医院神经内科,怀疑脑小血管病的患者; 3.进行MRI检查,并在一周内进行双眼免散瞳眼底彩色照相检查。

Inclusion criteria

1. Age >= 18 years old; 2. Patients suspected of cerebral small vessel disease who visited the Department of Neurology, Beijing Hospital; 3. Underwent MRI examination and non-mydriatic fundus color photography of both eyes within one week.

排除标准:

1.既往动脉瘤病史,或发现有未处理动脉瘤(直径>3mm); 2.已确诊的中枢神经系统退行性疾病,如帕金森病、阿尔兹海默症,其他原因导致的脑损伤引起的痴呆; 3.既往诊断过TOAST分型中除小动脉性梗死,其他分型都排除; 4.神经感染免疫性的白质病变; 5.已确诊的精神性疾病; 6.合并其他影响眼底血管和视盘结构的眼科疾病,如视网膜血管发育异常、青光眼、先天性视盘异常等; 7.屈光间质严重混浊影响眼底彩照成像质量; 8.患有严重的器质性疾病,如恶性肿瘤等,预期生存时间<2年; 9.孕妇或没有采取有效的避孕措施且没有妊娠试验记录的育龄期女性。

Exclusion criteria:

1. History of aneurysm or presence of an untreated aneurysm (diameter >3mm); 2. Confirmed degenerative central nervous system disorders, such as Parkinson's disease, Alzheimer's disease, or dementia caused by brain injury from other causes; 3. Exclusion of all TOAST subtypes except small artery disease; 4. Neuroinfectious or immune-mediated leukoencephalopathies; 5. Confirmed psychiatric disorders; 6. Concurrent ophthalmic conditions affecting retinal vascular or optic disc structures, such as retinal vascular dysplasia, glaucoma, or congenital optic disc abnormalities; 7. Severe opacity of the refractive media compromising the quality of fundus colour photography; 8. Presence of severe organic diseases, such as malignant tumours, with an expected survival time of <2 years; 9. Pregnant women or women of childbearing age not using effective contraception and lacking pregnancy test records.

CSVD诊断金标准为CSVD负荷评分(总分为0~4分),依据MRI检查结果评定,包含4项影像学标志:1.?近期皮质下小梗死(腔隙);2.?脑白质高信号(Fazekas评分);3.?脑微出血;4.?血管周围间隙扩大

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The gold standard for diagnosing CSVD is the CSVD total burden score (with a total score of 0-4 points), which is assessed based on MRI findings and includes 4 imaging markers: 1. Recent subcortical small infarcts (lacunes); 2. White matter hyperintensities (Fazekas score); 3. Cerebral microbleeds; 4. Perivascular spaces enlargement.

基于人工智能技术量化分析的免散瞳眼底彩照,提取视网膜血管管径、弯曲度、动静脉比、分形维数、分支夹角、血管密度等形态特征,融合受试者年龄、性别、BMI、血压、糖尿病史等人口学及临床危险因素,构建的脑小血管病(CSVD)风险预测模型。

Index test:

The CSVD risk prediction model constructed by quantitatively analyzing non-mydriatic fundus photographs based on artificial intelligence technology to extract retinal vascular morphological features such as vascular diameter, tortuosity, arteriovenous ratio, fractal dimension, branching angle and vascular density, and integrating demographic and clinical risk factors of subjects including age, gender, BMI, blood pressure and diabetes history.
