研究目的：

探讨不同25-羟维生素D水平的青少年抑郁障碍住院患者维生素D增效作用，并通过评估血清25(OH)D、白细胞介素-6（IL-6）、C反应蛋白（CRP）、5-羟色胺（5-HT）、抗抑郁药血药浓度及血脂水平等，阐述其可能得作用机制和安全性，以期为了解不同剂量维生素D对抗抑郁药的影响提供理论依据。  

Objectives of Study：

This study investigates the adjunctive efficacy of vitamin D in adolescent inpatients with depressive disorder who have different baseline levels of 25-hydroxyvitamin D. By assessing serum 25(OH)D, interleukin-6 (IL-6), C-reactive protein (CRP), 5-hydroxytryptamine (5-HT), antidepressant blood concentrations, and blood lipid profiles, we aim to elucidate its potential mechanisms of action and safety profile. The objective is to provide a theoretical foundation for understanding the impact of varying vitamin D doses on antidepressant effects.

纳入标准：

①符合精神障碍诊断与统计手册（第五版）（DSM-5）重性抑郁障碍诊断的我院的住院患者；②年龄：12–18 岁；③ CDRS-R ≥40；④血清25（OH）D < 20 ng/ml，根据《中国儿童维生素D营养相关临床问题实践指南》分级分为不足组（≥12ng/ml）和缺乏组（<12 ng/ml）

Inclusion criteria

1 Inpatients admitted to our hospital who meet the diagnostic criteria for Major Depressive Disorder according to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5); 2 Aged 12-18 years; 3 With a Children's Depression Rating Scale-Revised (CDRS-R) score of >= 40; 4 With a serum 25-hydroxyvitamin D [25(OH)D] level of < 20 ng/ml. Based on the criteria outlined in the "Practical Guide for Clinical Issues Related to Vitamin D Nutrition in Chinese Children," these patients were further stratified into an insufficiency group (>= 12 ng/ml) and a deficiency group (<12 ng/ml).

排除标准：

①符合DSM-5诊断标准的其他精神疾病，如精神分裂症、双相抑郁、物质使用障碍或其他精神障碍等；②合并感染、免疫疾病或其他严重躯体疾病；③影响维生素D代谢的疾病（结节病、甲状旁腺功能减退、甲状旁腺功能亢进、高钙血症、肾结石、肾脏衰竭或其他影响肾功能的疾病）；④妊娠者；⑤严重自杀风险；⑥近期曾服用含维生素D的相关药品或保健品；⑦智力障碍不能配合评估者；⑧已知对维生素D或其制剂辅料过敏。

Exclusion criteria：

1 Patients diagnosed with other psychiatric disorders per DSM-5 criteria, such as schizophrenia, bipolar depression, substance use disorder, or other mental disorders; 2 Those with concurrent infections, autoimmune diseases, or other severe systemic medical conditions; 3 Diseases affecting vitamin D metabolism (e.g., sarcoidosis, hypoparathyroidism, hyperparathyroidism, hypercalcemia, kidney stones, renal failure, or other conditions impairing renal function); 4 Pregnant individuals; 5 Patients with a significant risk of suicide; 6 Recent use of vitamin D-containing medications or supplements; 7 Individuals with intellectual disability unable to cooperate with the assessment; 8 Known allergy to vitamin D or any excipients in its formulations.
