研究目的：

本研究旨在揭示肺癌脑转移灶中潜在的免疫应答靶点和具有免疫活性的多肽片段，从而为开发针对性强、个体化程度高的免疫治疗策略提供坚实的科学依据。通过对转录组测序与生物信息学分析相结合识别肺癌脑转移特异性的新抗原肽段，并以此为基础，研发个体化的免疫疫苗。研究过程中，首先通过对转录组数据的深入分析，筛选出与肺癌脑转移相关的潜在新抗原肽段，这些肽段可作为特异性靶点引发针对脑转移灶的免疫反应。将这些新抗原肽段加载于树突状细胞（DCs）上，制备个体化的疫苗，并与免疫佐剂（如CpG）共同使用，通过增强抗肿瘤T细胞应答来提升疫苗的免疫效果。其次，通过优化树突状细胞疫苗的抗原加载与递呈策略，提升疫苗的免疫效果，使其在脑转移治疗中具有更大的临床应用潜力。最后，研究的最终成果将为肺癌脑转移的免疫治疗提供候选抗原和有效免疫治疗组合方案，推动其在临床中的转化应用。  

Objectives of Study：

This study aims to reveal potential immune response targets and immunogenic peptide fragments in brain metastases of lung cancer, thereby providing a solid scientific basis for the development of targeted and highly personalized immunotherapy strategies. By combining transcriptome sequencing with bioinformatics analysis, we identify novel antigenic peptides specific to brain metastases of lung cancer. Based on this, personalized immunovaccines are developed. During the research process, firstly, potential novel antigenic peptides related to brain metastases of lung cancer are screened through in-depth analysis of transcriptome data. These peptides can serve as specific targets to trigger an immune response against brain metastases. These novel antigenic peptides are loaded onto dendritic cells (DCs) to prepare personalized vaccines, which are then used in combination with immune adjuvants (such as CpG) to enhance the immune effect of the vaccine by boosting anti-tumor T cell responses. Secondly, by optimizing the antigen loading and presentation strategies of dendritic cell vaccines, the immune effect of the vaccine is enhanced, making it have greater clinical application potential in the treatment of brain metastases. Finally, the ultimate outcome of the study will provide candidate antigens and effective immunotherapy combination schemes for the immunotherapy of brain metastases of lung cancer, promoting its translational application in clinical practice.

纳入标准：

前瞻性收集脑转移瘤患者生物样本和临床信息 入选标准 复旦大学附属华山医院神经外科收治的脑肿瘤患者中，凡满足以下条件者均可入组： 1）年龄大于18岁，性别男女不限； 2）术前影像学怀疑是肺癌脑转移； 3）需要接受手术； 4）术中冰冻切片病理回报结果为肺癌脑转移； 5）在保证不影响临床常规诊疗所需的基础上，可取一定量新鲜生物样本用于研究； 6）自愿签署知情同意书。 回顾性收集肺癌脑转移原发灶和转移灶生物样本和临床信息 入选标准 复旦大学附属华山医院病理科诊断的肺癌脑转移病例中，凡满足以下条件者均可入组： 1）年龄大于18岁，性别男女不限； 2）病理诊断为肺癌脑转移且有原发灶石蜡样本； 3）在保证不影响临床常规诊断的基础上，可取石蜡切片（5片）进行免疫组化染色； 4）患者自愿加入本研究。

Inclusion criteria

Proactively collect biological samples and clinical information from patients with brain metastases Inclusion criteria Among the brain tumor patients admitted to the Department of Neurosurgery at Huashan Hospital affiliated to Fudan University, those who meet the following criteria can be enrolled: 1) Aged over 18, with no gender restrictions; 2) Preoperative imaging suspected lung cancer with brain metastasis; 3) Requires surgery; 4) The intraoperative frozen section pathology report revealed a brain metastasis of lung cancer; 5) A certain amount of fresh biological samples can be taken for research on the basis of ensuring that it does not affect the needs of clinical routine diagnosis and treatment; 6) Voluntarily sign the informed consent form. Retrospectively collect biological samples and clinical information from primary and metastatic foci of brain metastases in lung cancer Inclusion criteria Among the cases of lung cancer with brain metastasis diagnosed by the Department of Pathology at Huashan Hospital affiliated to Fudan University, all those who meet the following criteria can be enrolled: 1) Aged over 18, with no gender restrictions; 2) Pathological diagnosis of lung cancer with brain metastasis and paraffin samples of the primary lesion; 3) On the basis of ensuring that it does not affect clinical routine diagnosis, paraffin sections (5 slices) can be taken for immunohistochemical staining; 4) The patient voluntarily joins this study.

排除标准：

前瞻性收集脑转移瘤患者生物样本和临床信息 排除标准 患者如果符合以下任意一项标准，将不能入组本研究： 1）术中冰冻切片病理回报结果不符合肺癌脑转移； 2）除去正常诊疗所需的样本后，剩余样本量不够用于研究； 3）患者不愿意加入研究。 回顾性收集肺癌脑转移原发灶和转移灶生物样本和临床信息 排除标准 患者如果符合以下任意一项标准，将不能入组本研究： 1）蜡块存放过久发霉者； 2）不愿参加本研究者

Exclusion criteria：

Proactively collect biological samples and clinical information from patients with brain metastases Exclusion criteria Patients who meet any of the following criteria will not be eligible for enrollment in this study: 1) The intraoperative frozen section pathology report does not indicate brain metastasis of lung cancer; 2) After excluding the samples required for normal diagnosis and treatment, the remaining sample size is insufficient for research; 3) The patient is unwilling to participate in the study. Retrospectively collect biological samples and clinical information from primary and metastatic foci of brain metastases in lung cancer Exclusion criteria Patients who meet any of the following criteria will not be eligible for enrollment in this study: 1) Wax blocks that have been stored for too long and have become moldy; 2) Those who are unwilling to participate in this study

Data collection was completed by the researcher. When screening patients, the researcher timely filled the basic information of patients and other data into the Case Record Form (CRF), and patients meeting the inclusion and exclusion criteria are eventually included in this study. Data should come from and be consistent with original documents such as original records and/or laboratory inspection reports. Any observation and inspection results in the study should be timely, correct, complete, clear, standardized and true. The data administrator is responsible for reviewing and managing the entered data. For questions about the data, the data administrator will send corresponding questions to the researcher. The researcher will reply to the questions sent by the data administrator in time, and the data administrator can question again when necessary.
