研究目的：

本研究主要探讨盐酸右美托咪定鼻喷剂对膝髋关节置换手术患者腰麻成功率和患者满意度的影响，为临床合理用药提供依据。进一步证实右美托咪定鼻喷剂可以增加患者膝髋关节手术腰麻成功率及患者满意度，降低医疗成本，减少围术期并发症的发生，提高生活质量  

Objectives of Study：

This study mainly explores the impact of the Dexmedetomidine Hydrochloride Nasal Spray on the success rate of spinal anesthesia and patient satisfaction in patients undergoing knee and hip joint replacement surgery, providing a basis for rational clinical drug use. It further confirms that the right nasal spray can increase the success rate of spinal anesthesia and patient satisfaction in knee and hip joint surgeries, reduce medical costs, decrease the occurrence of perioperative complications, and improve the quality of life.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）ASA I-III级； （2）年龄50-80岁； （3）男女不限； （4）BMI18.5-29 kg/m^2； （5）择期于青岛市市立医院拟行膝髋关节置换术的患者。

Inclusion criteria

(1) ASA I-III grade; (2) Age range: 50 - 80 years old; (3) Gender: Open to both males and females; (4) BMI: 18.5 - 29 kg/m^2; (5) Patients scheduled to undergo knee and hip joint replacement surgery at Qingdao Municipal Hospital on an elective basis.

排除标准：

（1）患者依从性差，不能配合手术者; （2）有严重器质性病变（冠心病、支气管哮喘、重度高血压、严重的血液系统功能障碍、肝肾功能、电解质明显异常患者）； （3）II度及以上房室传导阻滞，心动过缓（HR<50次/分），低血压（SBP<90mmHg）； （4）有神经系统疾病史；慢性疼痛病史；药物成瘾及酗酒；长期应用阿片类药物史； （5）对盐酸右美托咪定鼻喷剂或其中的组分过敏患者； （6）文化水平低，不能满足相关评分量表调查者； （7）鼻部手术史，严重鼻炎、或存在鼻部结构异常的患者（如鼻中隔编曲）； （8）近4周内参加过其他药物的临床试验者； （9）穿刺部位感染，腰部手术史，脊柱严重损伤，凝血功能障碍或先天发育异常（如脊柱侧弯等）以及其他不适宜腰麻的情况或腰麻禁忌症。

Exclusion criteria：

(1) Patients with poor compliance and inability to cooperate with the surgery; (2) Patients with severe organic diseases (coronary heart disease, bronchial asthma, severe hypertension, severe blood system dysfunction, liver and kidney function, electrolyte abnormalities) (3) Second-degree or higher atrioventricular conduction block, bradycardia (HR < 50 beats per minute), hypotension (SBP < 90 mmHg); (4) History of neurological diseases; chronic pain history; drug addiction and alcohol abuse; long-term use of opioid drugs history; (5) Patients allergic to the nasal spray of dexmedetomidine hydrochloride or its components; (6) Low educational level, unable to complete the survey of the relevant scoring scale; (7) History of nasal surgery, severe rhinitis, or abnormal nasal structure (such as deviation of the nasal septum); (8) Participated in other drug clinical trials within the past 4 weeks; (9) Infection at the puncture site, history of lumbar surgery, severe spinal injury, coagulation dysfunction or congenital developmental abnormalities (such as scoliosis), and other conditions not suitable for spinal anesthesia or contraindications to spinal anesthesia.

首次使用前，需要预喷7喷进行鼻喷泵的激活。由鼻腔喷入，两侧鼻孔各喷入1喷，停留30s后再于两侧鼻孔各喷入1喷，总共给药4喷，共计100μg，建议给药在2min内完成

Spray into the nasal cavity, each side of the nostril should receive 1 spray, stay for 30 seconds, then spray another 1 spray into each side of the nostril. A research coordinator was designated to be responsible for preserving and distributing random numbers, preparing drugs, and coordinating information among researchers; a chief physician was appointed to be in charge of anesthesia implementation and recording intraoperative information; a deputy chief pharmacist was assigned to be responsible for administering study drugs postoperatively and monitoring drug safety; trained follow-up personnel were responsible for patient follow-up. The above researchers were unaware of each other's research results during the study period. Statistical analysis was completed by the Department of Anesthesiology of Qingdao Municipal Hospital.
