本项目拟:1) 从脑脊液循环与清除多维度信息中寻找分流术敏感型患者潜在的标志物,利用新型机器学习算法构建 iNPH脑脊液分流术疗效预测模型并验证其效能;2) 采用纵向追踪技术结合中介分析揭示分流术后“神经血管屏障-类淋巴-硬脑膜淋巴管”网络的时空演变与脑脊液循环清除的关系,阐明二者的动态变化与手术预后的内在关联,从神经体液稳态层面构建术后监测体系,推动诊疗范式从脑室形态学干预向神经体液循环稳态重建转型。 |

Objectives of Study: |
This project aims to: 1) Search for potential markers of patients sensitive to shunt surgery from multi-dimensional information of cerebrospinal fluid circulation and clearance, construct an efficacy prediction model of iNPH cerebrospinal fluid shunt surgery using a new machine learning algorithm and verify its efficacy, providing an objective basis for the precise diagnosis and treatment of iNPH; 2) Employ longitudinal multimodal MRI tracking combined with mediation analysis to investigate the spatiotemporal evolution of the "neurovascular barrier–glymphatic system–dural lymphatic vessel" network after shunt surgery and its impact on CSF circulation and waste clearance. This will help clarify the intrinsic connections between these dynamic changes and surgical outcomes, enabling the establishment of a postoperative monitoring system based on brain fluid homeostasis. Ultimately, this approach aims to shift the diagnostic and therapeutic paradigm from ventricular morphological interventions to the restoration of neurofluid circulatory homeostasis. |

1.INPH 患者:符合《特发性正常压力脑积水临床管理中国指南-2022》及国外相关 iNPH 指南,经过神经内科或外科医生评估为临床可疑患者。 2. 正常老年人组:(1)年龄>=60 岁;(2)无 iNPH 临床三联征,即无渐进性不明原因的步态障碍、认知障碍、尿失禁等;(3)颅脑 MR 显示脑室大小正常。 |

Inclusion criteria |
1. INPH Group: Patients meeting the criteria outlined in the "Chinese Clinical Management Guidelines for Idiopathic Normal Pressure Hydrocephalus-2022" and corresponding international iNPH guidelines, assessed by a neurologist or neurosurgeon as clinically suspected cases. 2. Normal elderly group: (1) Age >=60 years; (2) Absence of the clinical triad of idiopathic normal pressure hydrocephalus, namely no progressive unexplained gait disturbance, cognitive impairment, or urinary incontinence; (3) Normal ventricular size on cranial MRI. |

Exclusion criteria: |
1. Individuals with contraindications for MRI examinations, such as metallic implants in the body, contrast agent allergies, or claustrophobia; 2.Patients with poor image quality who are difficult to cooperate with. |
