研究目的：

本课题拟自行研制综合性的中小学生常见疾病相关健康素养量表，以准确评估中小学生对常见疾病相关知识的获取、理解、辨析和运用，以维护和促进自身健康的能力。研究采用基于TTM模型的多维度、多阶段的健康教育干预策略，探索提升中小学生常见疾病相关的健康素养水平的有效途径，寻找相较于单纯的健康知识传播更能实现理想的健康教育和行为改变的方式。  

Objectives of Study：

This project aims to develop a comprehensive health literacy scale for common diseases among primary and secondary school students, in order to accurately evaluate their ability to acquire, understand, and apply knowledge related to common diseases, and to maintain and promote their own health. The study adopts a multidimensional and multi-stage health education intervention strategy based on the TTM model to explore effective ways to improve the health literacy level of common diseases among primary and secondary school students.

纳入标准：

参与者需同时满足以下条件： 1.小学四、五年级以及初中一、二年级学生。 2.目前在所选学校注册并在校学习，且预期在研究随访期（约 1 年）内不毕业、转学或长期离校。 3.能够理解并完成量表及相关调查内容，具备完成评估所需的基本认知与能力。

Inclusion criteria

Participants must meet the following conditions simultaneously: 1. Students are in grades four and five of elementary school, and grades one and two of junior high school. 2. Students Currently have enrolled and attending the selected school, with no expectation of graduation, transfer, or long-term absence during the research follow-up period (approximately 1 year). 3. Students are able to understand and complete scales and related survey content, possessing the basic cognition and capabilities required to conduct assessments.

排除标准：

存在下列任意一项者将被排除： 1.存在严重慢性疾病、重大器质性疾病或影响日常学习活动的健康问题。 2.已知将在研究随访期内转学、长期外出或无法持续参与。 3.在入组前或随访期间，已参加或计划参加与本研究内容相似的系统性健康教育或行为干预项目。 4.存在明显认知、语言或行为障碍，即使在协助下仍无法完成量表或体格评估。

Exclusion criteria：

Any of the following will be excluded: 1. There are serious chronic diseases, major organic diseases, or health problems that affect daily learning activities. 2. It is known that there will be a transfer, long-term absence, or inability to continue participating during the follow-up period of the study. 3. Prior to enrollment or during follow-up, the student has participated or plan to participate in systematic health education or behavioral intervention projects similar to the content of this study. 4. There are obvious cognitive, language, or behavioral disorders, and even with assistance, the scale or physical assessment cannot be completed.
