研究目的：

本研究以外周生物力学和中枢效应为切入点，通过使用表面肌电图与功能性近红外脑功能成像技术，明确运动训练对健康患者与慢性非特异性下腰痛患者的外周肌肉机械特性与中枢脑激活的差异化表现，并通过讨论cNSLBP的外周生物力学及中枢调节机制，进一步明确病因及发病机制，为预测腰痛的发生及制定更有针对性、更有效的下腰痛康复治疗方案提供科学依据。  

Objectives of Study：

This study takes peripheral biomechanics and central effects as the starting point. By using surface electromyography and functional near-infrared spectroscopy for brain function imaging, it clarifies the differential manifestations of exercise training on the peripheral muscle mechanical properties and central brain activation between healthy patients and those with chronic nonspecific low back pain (cNSLBP). Furthermore, by discussing the peripheral biomechanics and central regulatory mechanisms of cNSLBP, it further clarifies the etiology and pathogenesis, providing a scientific basis for predicting the occurrence of low back pain and formulating more targeted and effective rehabilitation treatment plans for low back pain.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.慢性非特异性腰痛（cNSLBP）组： 1.1.年龄 18-60 岁; 1.2.在过去 3 个月内被诊断为慢性非特异性腰痛，且 / 或在过去 6 个月中至少有一半时间存在 T12 至臀褶之间的疼痛; 1.3.过去 3 个月内最严重的疼痛在视觉模拟量表 （VAS） 上疼痛强度≥3 分; 1.4.右利手 。 2.健康对照（HC）组： 2.1.年龄 18-60 岁; 2.2.至少过去 3 个月没有腰痛病史； 2.3.右利手。

Inclusion criteria

1. Chronic Nonspecific Low Back Pain (cNSLBP) group: 1.1. Aged 18-60 years; 1.2. Diagnosed with chronic nonspecific low back pain within the past 3 months, and/or having pain between T12 and the gluteal fold for at least half of the past 6 months; 1.3. The most severe pain in the past 3 months has a pain intensity of ≥3 points on the Visual Analogue Scale (VAS); 1.4. Right-handed. 2. Healthy Control (HC) group: 2.1. Aged 18-60 years; 2.2. No history of low back pain for at least the past 3 months; 2.3. Right-handed.

排除标准：

以下为健康对照组与cNSLBP组共同排除标准： 1.1.过去3个月内接受了腰椎、下肢或腹部手术; 1.2.神经系统疾病(如多发性硬化症、帕金森氏病伴有可能干扰痛觉的神经症状等); 1.3.恶性肿瘤、脑部手术或脑肿瘤病史; 1.4.近红外脑功能扫描禁忌症(头皮病变、金属植入物、头部外伤等); 1.5.存在神经根病、腰椎管狭窄或脊椎滑脱体征; 1.6.妊娠期或经期痛经发作 1.7.3日内服用过止痛药。

Exclusion criteria：

The following are the common exclusion criteria for the healthy control group and the cNSLBP group: 1. Having undergone lumbar spine, lower extremity, or abdominal surgery within the past 3 months; 2. Neurological diseases (such as multiple sclerosis, Parkinson's disease with neurological symptoms that may interfere with pain perception, etc.); 3. A history of malignant tumors, brain surgery, or brain tumors; 4. Contraindications to near-infrared brain function scanning (scalp lesions, metal implants, head trauma, etc.); 5. Presence of signs of radiculopathy, lumbar spinal stenosis, or spondylolisthesis; 6. Pregnancy or dysmenorrhea during menstruation; 7. Having taken painkillers within 3 days.
