研究目的：

（1）探索传统子宫肌瘤切除术与HIFU消融术后不良妊娠结局的发生率差异。 （2）识别影响妊娠结局的关键因素（手术方式、肌瘤位置、数量、大小等）。 （3）基于多中心的随机对照研究获取不同术式的生殖保护效应证据，形成基于生育力保护的手术方式选择临床路径。  

Objectives of Study：

(1) To investigate the differences in the incidence of adverse pregnancy outcomes between traditional myomectomy and HIFU ablation. (2) To identify key factors influencing pregnancy outcomes (surgical approach, fibroid location, number, size, etc.). (3) To obtain evidence on the fertility preservation effects of different procedures through a multicenter randomized controlled study, and develop a clinical pathway for selecting surgical approaches based on fertility preservation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄20-45岁的女性 ②单发肌瘤直径4-15cm或多发肌瘤最大直径≤10cm ③接受HIFU或手术（腹腔镜/开腹/宫腔镜）治疗； ④接受治疗后持续妊娠到28周并活胎孕妇

Inclusion criteria

1 Women aged 20-45 years 2 Single fibroid diameter of 4-15 cm or multiple fibroids with the largest diameter <=10 cm 3 Underwent HIFU or surgical (laparoscopic/open/hysteroscopic) treatment 4 Women who maintained pregnancy beyond 28 weeks with a viable fetus after treatment

排除标准：

① 非生育年龄女性 ② 0型粘膜下子宫肌瘤 ③ 合并子宫内膜异位症/腺肌症/输卵管积水等妊娠高危因素 ④ 恶性肿瘤或交界性肿瘤

Exclusion criteria：

1 Non-reproductive age women 2 Type 0 submucosal uterine fibroids 3 Coexisting high-risk factors for pregnancy such as endometriosis/adenomyosis/hydrosalpinx 4 Malignant or borderline tumors

This study employed a block randomization method, stratified by research center, with randomized use of block sizes of 6, 8, and 10 within each stratum.
