研究目的：

本项目旨在构建超声引导竖脊肌平面阻滞（ESPB）联合喉罩全麻的高原腹腔镜胆囊切除术围术期管理策略，重点解决以下临床问题：1镇痛优化：高原低氧环境使患者对阿片类药物呼吸抑制敏感性增高，ESPB可减少70%术中阿片用量，降低术后呼吸抑制风险；2.器官保护：通过阻断手术应激反应，减轻高原低氧与手术创伤叠加导致的肝、肾、膈肌功能损伤；项目实施将推动西藏地区围术期麻醉管理的标准化，为高海拔地区外科手术提供循证医学证据。  

Objectives of Study：

This project aims to establish a perioperative management strategy for laparoscopic cholecystectomy in Xizang using ultrasound-guided erector spinae plane block (ESPB) combined with endotracheal intubation general anesthesia. The key issues to be addressed include: 1. Pain optimization: The hypoxic environment at high altitudes makes patients more sensitive to respiratory depression caused by opioids. ESPB can reduce the intraoperative opioid dosage by 70% and lower the risk of postoperative respiratory depression; 2. Organ protection: By blocking the surgical stress response, it can alleviate the combined damage to the liver, kidneys, and diaphragm caused by high-altitude hypoxia and surgical trauma. The implementation of this project will promote the standardization of perioperative anesthesia management in Xizang and provide evidence-based medical evidence for surgical procedures in high-altitude areas.

纳入标准：

①　年龄18-65岁， ②　BMI 18.5-28 kg/m2 ③　西藏常住居民（海拔＞3000m居住≥6个月） ④　确诊的胆囊结石患者 ⑤　拟行腔镜胆囊切除术 ⑥　静息SpO? ≥85%（高原环境） ⑦　ASA分级Ⅰ-Ⅲ级

Inclusion criteria

1. Age: 18 - 65 years old; 2. BMI: 18.5 - 28 kg/m^2; 3. Permanent residents of Xizang (residing at an altitude of > 3000m for >= 6 months); 4. Patients diagnosed with gallbladder stones; 5. Scheduled for laparoscopic cholecystectomy; 6. Resting SpO2 >= 85% (in high-altitude environment); 7. ASA classification: I - III grade

排除标准：

①　麻醉相关禁忌 (1)存在局麻药过敏史或酰胺类局麻药使用禁忌； (2)穿刺区域存在感染、畸形或占位性病变； (3)凝血功能异常（INR＞1.5 或 PLT＜80×10?/L）； (4)严重神经系统疾病（如脊髓损伤、多发性神经病变）。 ②　手术相关禁忌 (1)既往有上腹部复杂手术史（影响解剖标志识别）； ③　高原生理代偿障碍 (1)静息未吸氧状态下SpO?＜80%（提示重度低氧血症）； (2)急性高原病评分（AMS）＞5分（参照Lake Louise标准）； (3)红细胞比容（Hct）＞65%（高原红细胞增多症未控制）。 ④　其他排除因素 (1)长期使用阿片类药物（＞30mg/日吗啡等效剂量）； (2)精神疾病或认知障碍（无法配合疼痛评估）； (3)孕妇或哺乳期妇女； (4)研究者判断不适宜入组的其他情况。

Exclusion criteria：

1. Anesthesia-related contraindications (1). History of local anesthetic allergy or contraindications to the use of amide-type local anesthetics; (2). Presence of infection, deformity or space-occupying lesion in the puncture area; (3). Abnormal coagulation function (INR > 1.5 or PLT < 80 × 10^9/L); (4). Severe neurological diseases (such as spinal cord injury, multiple neuropathies). 2. Surgery-related contraindications (1). Previous history of complex upper abdominal surgery (affecting the identification of anatomical landmarks); 3. Physiological compensatory disorders at high altitudes (1). SpO2 < 80% at rest without oxygen inhalation (indicating severe hypoxemia); (2). Acute mountain sickness score (AMS) > 5 points (refer to the Lake Louise standard); (3).Hematocrit (Hct) > 65% (uncontrolled high-altitude polycythemia). 4. Other exclusion factors (1). Long-term use of opioid drugs (>30mg/day morphine equivalent dose); (2). Mental illness or cognitive impairment (unable to cooperate with pain assessment); (3).Pregnant or lactating women; (4).Other situations judged by the researchers as not suitable for inclusion.

The allocation numbers and the encoded groups were sealed in opaque envelopes and were opened in sequence by independent anesthesiologists who performed the regional block and were not involved in the result evaluation (without blinding). The enrolled patients were blinded; the anesthesiologists responsible for the block effect and the postoperative result evaluation were also blinded to the treatment and the allocation groups; the intraoperative anesthesia managers and the researchers responsible for the result statistics were also blinded.
