研究目的:

研究一种微导管辅助冲洗方法,降低经输尿管硬镜钬激光碎石术手术区域的温度和压力。  

Objectives of Study:

To study a microcatheter-assisted irrigation method to reduce the temperature and pressure in the area of transureteral holmium laser lithotripsy.

药物成份或治疗方案详述:

使用一种输尿管导管作为辅助冲洗的微导管,并将其引入输尿管镜的内腔,通过微导管导管加压注入26℃0.9%NaCl溶液,液体通过输尿管镜内腔与微导管之间的间隙回流出输尿管镜,形成连续冲洗。通过实验模型,在液体灌注压强分别为150mmHg、200mmHg、450mmHg、人工自由推注的不同条件下,测定钬激光输出功率在0.6J×20Hz、0.8J×20Hz、1.0J×20Hz情况下,模拟经输尿管镜输尿管碎石过程中碎石区域温度及压力变化。得到能保持局部温度低于43度,压力低于20mmHg(27cmH2O)的微导管型号和灌注条件。采用研究结果所推荐的微导管型号及灌注条件进行临床研究,观察临床应用效果。 

Description for medicine or protocol of treatment in detail:

A ureteral catheter was used as a microcatheter to assist flushing, and it was introduced into the lumen of the ureteroscope. The microcatheter was pressurized to inject 0.9% NaCl solution at 26 ° C. The liquid flowed back out of the ureteroscope through the gap between the lumen of the ureteroscope and the microcatheter to form a continuous flushing. Through the experimental model, under different conditions of liquid perfusion pressure of 150mmHg, 200mmHg, 450mmHg, and artificial free perfusion, the output power of holmium laser was measured at 0.6J × 20Hz, 0.8J × 20Hz, and 1.0J × 20Hz. The temperature and pressure changes in the gravel area during ureteroscopic ureteral lithotripsy were simulated. The microcatheter model and perfusion conditions that can keep the local temperature below 43 degrees and the pressure below 20mmHg (27cmH2O) were obtained. The microcatheter model and perfusion conditions recommended by the research results were used for clinical research to observe the clinical application effect. 

纳入标准:

1.年龄18岁及以上,75岁以下。 2.经泌尿系统CT平扫确诊为中下段输尿管结石。 3.具有经尿道输尿管硬镜或输尿管软镜行上尿路结石激光碎石指征。 4.自愿参加本研究,愿意完成后续随访,并签署书面知情同意书。 5.符合《泌尿系结石诊疗指南》相关诊断标准,经CT检查确诊为输尿管结石。

Inclusion criteria

1.Age 18 years old and above, under 75 years old. 2.The middle and lower ureteral calculus was diagnosed by CT scan of the urinary system. 3.Laser lithotripsy of upper urinary tract stones was indicated by transurethral ureteroscopy or flexible ureteroscopy. 4.Volunteer to participate in this study, willing to complete follow-up follow-up, and sign a written informed consent form. 5.It met the relevant diagnostic criteria of the "Guidelines for the Diagnosis and Treatment of Urinary Stones" and was diagnosed as ureteral calculus by CT examination.

排除标准:

1.排除标准(Exclusion Criteria): 2.患有未治愈的肾结核或未控制的尿路感染。 3.合并泌尿系统恶性肿瘤。 4.术前影像学检查提示输尿管严重扭曲、输尿管狭窄以及其他原因可能导致进镜困难者。 5.髋关节存在异常,无法耐受截石位。 6.患有心脑血管等严重危及生命的合并症,不能耐受手术。 7.年龄<60岁且血压>=140/90 mmHg(1 mmHg=0.133 kPa),年龄<=60岁、不伴糖尿病且血压>=150/90 mmHg或合并糖尿病且血压>=140/90 mmHg。 8.血糖未控制的糖尿病患者,糖化血红蛋白>7.0%或空腹血糖>=9 mmol/L。 9.存在凝血功能异常、全身出血性疾病或筛选期3个月内有输血史者。 10.处于妊娠状态或6个月内计划妊娠及哺乳期女性。 11.近3个月内参加其他临床试验者。

Exclusion criteria:

1.Exclusion Criteria: 2.Have uncured kidney tuberculosis or an uncontrolled urinary tract infection. 3.Complicated with malignant tumor of urinary system. 4.Preoperative imaging examination indicates severe distortion of the ureter, stricture of the ureter, and other reasons that may lead to difficulty in access. 5.The hip joint is abnormal and cannot tolerate the lithotomy position. 6.Suffers from serious life-threatening comorbidities such as cardiovascular and cerebrovascular diseases, and cannot tolerate surgery. 7.Age < 60 years with blood pressure >=140/90 mmHg (1 mmHg = 0.133 kPa), age<=60 years without diabetes and blood pressure >=150/90 mmHg or with diabetes and blood pressure>= 140/90 mmHg. 8.In diabetic patients with uncontrolled blood glucose, glycated hemoglobin > 7.0% or fasting blood glucose >=9 mmol/L. 9.Those with abnormal coagulation function, systemic hemorrhagic disease, or a history of blood transfusion within 3 months of the screening period. 10.Women who are pregnant or planning to become pregnant and breastfeeding within 6 months. 11.Participants in other clinical trials within the past three months.
