研究目的：

复杂胫骨平台骨折是一种常见的高能量损伤骨折类型，并伴有膝关节周围广泛的软组织损伤，切开复位内固定术（ORIF）已经成为复杂胫骨平台骨折手术治疗的首选方案。术中隐性失血可导致严重后果，近年来被广泛关注。如何有效减少术中隐性失血，降低骨折患者因隐性失血所导致的各种并发症，值得进一步研究。目前氨甲环酸已被广泛用于膝髋关节置换术中控制失血量，取得满意的疗效。但尚未见在粗隆间骨折中应用的报道。在该研究中我们根据术中使用氨甲环酸静脉或局部方案对患者进行分组对照研究，观察氨甲环酸对ORIF术中隐性失血的影响，以达到减少手术失血、降低输血量的目的。  

Objectives of Study：

Complex tibial plateau fractures are a common type of high-energy injury fractures, and are accompanied by extensive soft tissue injuries around the knee. Open reduction and internal fixation (ORIF) has become the first choice for surgical treatment of complex tibial plateau fractures. Invisible blood loss during surgery can lead to serious consequences and has been widely concerned in recent years. How to effectively reduce hidden blood loss during operation and reduce various complications caused by hidden blood loss in patients with fractures is worthy of further study. Tranexamic acid has been widely used in knee and hip arthroplasty to control blood loss and achieve satisfactory results. However, no reports have been reported on the application of intertrochanteric fractures. In this study, we conducted a group-controlled study of patients based on intraoperative tranexamic acid intravenous or local regimen to observe the effect of tranexamic acid on hidden blood loss during ORIF surgery to reduce surgical blood loss and reduce blood transfusion .

纳入标准：

年满 18 周岁以上
伤后3天内入院复杂胫骨平台骨折患者，拟接受手术治疗

Inclusion criteria

18 years old and above
Patients with complicated tibial plateau fracture admitted to hospital within 3 days after injury

排除标准：

受伤时间大于24小时入院患者
住院时间小于48小时
临床明显活动性出血
已知的凝血异常（如血友病）
需要持续抗凝治疗的患者（如机械瓣患者）
严重肝脏、肾脏疾病
有精神疾病等，预期依从性不良的患者
有抗凝药物使用禁忌症

Exclusion criteria：

1. Injured patients admitted for more than 3 days;
2. Hospital stay less than 48 hours;
3. Clinically active bleeding;
4. Known coagulation abnormalities (such as hemophilia);
5. Patients requiring continuous anticoagulant therapy (eg, mechanical valve patients);
6. Severe liver and kidney disease;
7. There is mental illness, etc., and patients with poor compliance are expected;
8. There are contraindications to the use of anticoagulant drugs.
