研究目的：

（1）构建维持性血液透析患者透析中运动干预方案。 （2）评价运动干预方案对维持性血液透析患者的效果。  

Objectives of Study：

(1) Construct the exercise intervention program for maintenance hemodialysis patients. (2) To evaluate the effect of exercise intervention program on maintenance hemodialysis patients.

药物成份或治疗方案详述：

对照组：采用我院现行的“责任制整体护理”工作制度，为患者提供常规护理和健康教育。内容如下： （1）对照组患者由主治医生给予疾病知识、血液透析相关知识及注意事项指导。责任护士给予常规护理指导和健康教育，包括血液净化中心简介、病房环境、规章制度、血管通路维护、病情监测、饮食指导、用药指导及药品管理、跌倒预防及运动指导、家庭护理等。 （2）采取线下+线上结合的形式进行健康小讲座,由科室专科护士及健康教育指导师进行授课，每月一次,每次30-60分钟，内容包括血液透析基础知识、血管通路的日常护理、饮食指导、运动指导、用药指导、心理健康教育等。 实验组：在原有的“责任制整体护理”的基础上,遵循基于健康信念模式已构建的运动干预方案进行干预，具体实施方法如下： （1）组建运动干预实施团队：遵照已构建的干预方案，由专业的医护人员组成团队成员，包括研究者本人、血液净化中心护士长1名、主治医生1名、康复医生1名、国家级血液净化专科护士5名、研究生2名。组建团队后由研究者本人介绍研究目的、内容，并针对运动干预方案及相关应急预案进行培训，确保所有研究成员熟悉干预方案的实施过程。主治医生、康复医师及护士负责患者临床状况的综合评估、运动能力评估，运动处方的制定及调整。研究者本人及护士长负责制定培训计划，进行培训，监督方案的正确实施。5名国家级血液净化专科护士负责运动方案的实施及监测。研究者本人及两名研究生负责收集资料。 （2）运动前患者准备:由研究者对实验组患者进行培训，包括运动的安全性、运动的获益、血管通路的维护、充分透析的重要性、运动的适应症及禁忌症等，并获得患者/家属的知情同意，签署知情同意书。如患者出现以下情况，则停止运动：严重的高血压(血压超过180/110mmHg)，或低血压(＜90/60mmHg)；心肺疾病：严重的心力衰竭、心律失常，不稳定性心绞痛，重度心包积液、瓣膜狭窄，肥厚性心肌病，主动脉夹层等，未控制的肺动脉高压(肺动脉平均压＞55mmHg)；急性临床事件：急性全身炎症性疾病或发热、心脑血管疾病急性期、创伤性疾病急性期等；新发深静脉血栓的症状，如小腿不正常的发红和肿痛时要暂缓或停止运动；患者不能配合运动等。 （3）运动处方的实施： 运动频率：依照患者自身透析频率2～3次/周。 运动强度：由低强度的运动开始，循序渐进，逐步过渡到中等强度运动。采用主观运动强度等级量表(RPE)来测定运动强度，一般设置为12~14分即可即患者有点吃力，感觉到稍微有点累，但又可以轻松地与人交谈，没有吃力的状态。 运动时间：运动时机为上机后的30min到２h之间完成。目标运动时间30～60min，根据患者情况个性化选择1次或分次完成。 运动类型：运动干预团队制定以下两种运动方案，实验组患者可以根据自身情况及喜好选择以下方案之一。 方案一：采用透析中联合有氧、抗阻和柔韧性为一体的卧位体操运动锻炼，共分为9节，前七节动作每个循环4个八拍，后两节动作每个循环内持续两分钟，患者也可根据自身情况进行调整，比如逐渐增加次数和抗阻力的强度，循序渐进。 第一节：抓握运动 非动静脉内瘘测上肢伸直五指，缓慢握拳，缓慢松开； 第二节：上肢肘关节屈曲，伸展运动 非动静脉内瘘侧上肢肘关节缓慢屈曲，缓慢伸直； 第三节：上肢抬起、落下运动 非动静脉瘘侧上肢手臂伸直，手心朝上，缓慢抬起，缓慢落下； 第四节：上肢负重抬起落下运动 非动静脉瘘侧上肢负重0.5kg～1kg盐袋缓慢抬起，缓慢落下； 第五节：膝关节屈曲伸展运动 左右膝关节交替缓慢屈曲，缓慢伸直； 第六节：双下肢抬起落下运动 左右两腿交替，缓慢抬起，缓慢落下； 第七节：下肢负重抬起落下运动 双脚踝部各负重0.5kg～1kg盐袋，缓慢抬起，缓慢落下； 第八节：上肢肌力训练 非动静脉内瘘侧上肢手心紧握握力圈2分钟； 第九节：下肢肌力训练 左右足交替缓慢抬离床面30度，足部尽可能背伸，保持此姿势1～2分钟。 方案二：采用可调节阻力的仰卧位脚踏车运动，先进行5～10min的无阻力运动，然后根据自身肌力状况进行阻力运动，强度由小及大，时间由短及长，最后再进行5～10分钟的无阻力运动。 （4）运动的监测与评估：患者前两次运动由护士和医生共同监测，当患者掌握运动方法且稳定后由护士独自监测，由患者或护士将每次运动情况记录下来，包括运动时间、类型、强度等，康复师每月参与患者的评估和下一步方案的制定。运动前、中、后均需要监测，包括患者生命体征，血管通路等情况，加强患者透析中监测并进行运动强度评估。运动过程中密切监测患者有无任何不适，并查找原因，当患者出现以下情况，则立即停止运动，并由医生评估后进行相应的抢救措施，如持续胸背部疼痛、心悸、胸闷；明显气喘，交谈困难头痛、头晕、周身无力、黑曚、大汗；严重心律失常；运动相关的肌肉痉挛、关节疼痛等。另外，对于糖尿病患者应注意低血糖的风险，必要情况下监测其运动前、后血糖水平。 （5）干预时长：本研究干预时长为3个月。 

Description for medicine or protocol of treatment in detail：

Control group: The current "responsibility system of holistic nursing" in our hospital was adopted to provide routine nursing and health education for patients. The content is as follows: (1) Patients in the control group were instructed by the attending doctor about disease knowledge, hemodialysis related knowledge and matters needing attention. The nurse in charge gives routine nursing guidance and health education, including the introduction of blood purification center, ward environment, rules and regulations, vascular access maintenance, disease monitoring, diet guidance, medication guidance and drug management, fall prevention and exercise guidance, and home care. (2) Take the form of offline+online combination to give a small health lecture, which is taught by department nurses and health education instructors once a month for 30-60 minutes each time. The contents include basic knowledge of hemodialysis, daily care of vascular access, diet guidance, exercise guidance, medication guidance, mental health education, etc. Experimental group: On the basis of the original "responsibility-based holistic nursing", follow the exercise intervention plan based on the health belief model, and the specific implementation methods are as follows: (1) Establish a sports intervention implementation team: according to the established intervention plan, the team members are composed of professional medical staff, including the researcher himself, a head nurse in the blood purification center, an attending doctor and a rehabilitation doctor. 

纳入标准：

（1）终末期肾病患者：指各种慢性肾脏疾病的终末阶段，一般认为当肾小球滤过率降至15ml/（min.1.73m^2)以下时即可诊断,常伴有尿毒症的症状和体征，或需要进行肾脏替代治疗。 （2）年龄18～65 周岁； （3）稳定透析治疗>=3 个月，2～3 次/周，每次 4 个小时； （4）使用动静脉瘘透析； （5）病情稳定，心功能1～2级 （6）自愿参加本研究。

Inclusion criteria

(1) End-stage renal disease patients: refers to the final stage of various chronic kidney diseases, generally considered to be diagnosed when the glomer filtration rate drops below 15ml/(min.1.73m^2), often accompanied by symptoms and signs of uremia, or requiring renal replacement therapy (2) Age 18 to 65 years old; (3) Stable dialysis treatment >=3 months, 2-3 times/week each time for 4 hours; (4) Use of arteriovenous fistula for dialysis; (5) Stable condition, heart function grade 12 (6) Voluntary participation in this study.

排除标准：

（1）患有严重心脑血管疾病者； （2）严重的下肢血栓，脊柱、关节等疾病，不能完成运动； （3）有认知障碍及精神疾病的患者； （4）急性感染，手术和创伤中； （5）透析前收缩压>=180mm Hg和（或）舒张压>=100mm Hg （6）Hb<90g/L，Alb<=32g/L。

Exclusion criteria：

(1) those suffering from serious cardiovascular and cerebrovascular diseases; (2) Severe lower extremity thrombosis, diseases such as spine and joints, and inability to complete exercise; (3) Patients with cognitive impairment and mental illness; (4) Acute infection, operation and trauma; (5) Before dialysis, systolic blood pressure is >=180mm Hg and/or diastolic blood pressure is >=100mm Hg. (6)Hb<90g/L,Alb<=32g/L.

由研究者本人采用随机抽样法选取 2024年 7月至 2024年 9月在中国医科大学附属盛京医院血液净化中心接受 MHD 治疗的80例患者作为研究对象。采取随机数字表法将研究对象随机分为实验组和对照组 。每组患者各40例。即将选取的研究对象从1-80进行编号，即为每一个研究对象分配一个唯一的编号，再从随机数字表中任一行随机数字开始，比如第20行最左端开始横向连续取80个两位数，将80个随机数与研究对象按顺序一一对应，将80个随机数从小到大进行编号，如果随机数相等，则先出现的为小，按照事先设定的规则随机编号为1-40号的研究对象进入实验组，41-80号研究对象进入对照组。
