研究目的：

在人类发育的过程中，孕晚期至婴幼儿时期（＜3岁）大脑在功能和结构上均处于快速发育、高度可塑的关键阶段，也是大脑易受损伤的时间窗。由于全麻药物直接作用于中枢神经系统，这一药理特点让人们对其是否会影响发育脑功能展开了相关研究。但GAs产生全麻效应的工作机理至今仍未被完成阐明，一定程度上也限制了探索GAs对发育期大脑影响的研究进展。基于我国接受GAs的儿童日益增多，本试验对于GA介导的神经发育异常仍需进行深入的机制研究。  

Objectives of Study：

During human development, the late prenatal to early childhood period (less than 3 years old) is a critical stage of rapid brain development and high plasticity in both function and structure, as well as a vulnerable window for brain injury. Due to the direct action of general anesthetics on the central nervous system, this pharmacological characteristic has led to research on whether they may affect developing brain function. However, the mechanism of general anesthetics' effect on producing general anesthesia remains incompletely understood, which to some extent limits research progress on their impact on the developing brain. Given the increasing number of children receiving general anesthetics in China, this study requires further in-depth mechanistic research on GA-mediated neurodevelopmental abnormalities.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.3周岁以下，性别不限，接受单次超过3小时或次数≥3次的非体 外循环手术患儿（暴露组）。及年龄匹配未接受手术麻醉的患儿（对照组）； 2.除手术相关疾病外，无其他重大先天性疾病或遗传性代谢性疾病； 3出生 胎龄≥37周和<42周，单胎和非试管婴； 4.非小于胎龄儿； 5.家庭背景、生活环境等信息相对稳定且可获取； 6.ASA分级≤III级；

Inclusion criteria

1.Children under 3 years old, regardless of gender, who have undergone non-extracorporeal circulation surgery with either a single procedure lasting over 3 hours or >=3 procedures (exposure group), and age-matched children who have not received surgical anesthesia (control group). 2.Apart from surgery-related diseases, there are no other major congenital diseases or hereditary metabolic disorders. 3.Born at gestational age >=37 weeks and <42 weeks, singleton and non-IVF. 4.Not small for gestational age (NSGA). 5.Family background, living environment and other information are relatively stable and accessible. 6.ASA classification <=III.

排除标准：

1.出生时诊断为高危儿；
2.接受麻醉前已存在神经发育和认知发育障碍诊断；
3.合并有肝、肾、心脏等系统性全身慢性病；
4.确诊肿瘤患儿；
5.出于任何原因不能配合研究；
6.在入选研究前的3个月内服用其他试验药或参与了其他临床试验；
7.研究者认为不宜纳入本实验的患儿；
8.行神经系统相关手术的患儿；

Exclusion criteria：

1.Diagnosed as a high-risk infant at birth.
2.A diagnosis of pre-existing neurodevelopmental and cognitive disorders prior to anesthesia.
3.With systemic chronic diseases including liver, kidney, and heart conditions.
4.Child diagnosed with tumor.
5.For any reason unable to cooperate with the research.
6.Taken other investigational drugs or participated in other clinical trials within 3 months prior to enrollment in the study.
7.Children who are considered unsuitable for inclusion in this study by researchers.
8.Children undergoing neurological system-related surgery.

采集：术后1年由具有资质的小儿保健医生行Gessell量表。 管理：利用 Epidata3.0 软件建立调查问卷数据库，并采用双人双录入的方式进行问卷录入，对录入数据库信息进行一致性比对，针对不一致的数据由第三人对其进行复核录入，在核对准确无误后固定数据库。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data Collection: The Gessell Developmental Schedules were administered by qualified pediatric healthcare professionals one year after surgery. Data Management: A questionnaire database was established using Epidata3.0 software, and data entry was conducted by two persons independently. The entered data were compared for consistency, and any discrepancies were reviewed and corrected by a third person before finalizing the database.
