研究目的：

（1）循证肺移植患者术后早期离床管理方案 （2）构建并验证基于CUSP理论框架下肺移植患者术后早期安全离床管理模式  

Objectives of Study：

(1) Evidence-based early out-of-bed management plan for patients after lung transplantation (2) Construct and verify the early safe out-of-bed management model for lung transplant patients based on the CUSP theoretical framework

纳入标准：

1.符合 ISHLT 指南标准的终末期肺病者； 2.首次行肺移植手术的患者； 3.年龄≥18 岁； 4.自愿参与本研究。

Inclusion criteria

1.Patients with end-stage lung disease who meet the criteria of the ISHLT guidelines; 2.Patients undergoing lung transplantation surgery for the first time; 3.Age >=18 years old; 4.Voluntarily participate in this research;

排除标准：

1.多器官联合移植患者； 2.肢体瘫痪、骨折等影响肢体活动能力者； 3.合并心、脑、肾等严重疾病者； 4.抑郁症等精神病及阿尔兹海默病等认知障碍者； 5.代谢性、中毒性、颅内感染、镇静剂过量等致精神异常者。

Exclusion criteria：

1.Patients undergoing combined multi-organ transplantation;
2.Those with limb paralysis, fractures or other conditions that affect their limb mobility;
3.Those with severe diseases of the heart, brain, kidneys, etc;
4.People with mental illnesses such as depression and cognitive impairments such as Alzheimer's disease;
5.Those with mental disorders caused by metabolic, toxic, intracranial infections, excessive sedatives, etc;

主管医生评估患者术后生命体征、下肢肌力情况等术后整体恢复情况，决定患者术后首次下床活动时间；患者术后首次下床活动由康复师主导，多学科协同，照顾者参与，辅助患者下床，护士通过及时病情监测、适时检验检查、精准药物治疗、平衡液体容控、有效感染防控、个性营养干预、廓清气道管理、实时康复训练、调适心理认知等策略实施参与离床管理。

The attending physician assesses the patient's postoperative vital signs, lower limb muscle strength, and overall recovery status, and determines the time for the patient's first postoperative ambulation. The patient's first postoperative ambulation is led by the rehabilitation therapist, with the participation of multiple disciplines and the involvement of caregivers. The nurse implements the management of the patient's ambulation through strategies such as timely condition monitoring, timely laboratory tests and examinations, precise drug treatment, balanced fluid volume control, effective infection prevention and control, individualized nutritional intervention, airway clearance management, real-time rehabilitation training, and psychological adjustment.
