研究目的：

通过对老年牙周炎患者的口腔自我管理现状及影响因素研究，结合文献回顾分析，在此基础上构建基于ITHBC老年牙周炎患者口腔自我管理方案并进行验证，为我国老年口腔健康管理提供了理论基础，改善和促进老年患者的口腔健康，提升老年患者的生活质量，为响应国家“一老一小”，加快口腔预防“梁柱”建设，筑牢基层口腔预防网络，让口腔“共健共富”。为实现全生命周期的管理，实现健康中国提供参考依据。 （1）通过问卷调查及基线检查，了解老年牙周炎患者的口腔卫生状况、对口腔健康知识的认知以及自我管理水平； （2）本研究基于ITHBC为指导，构建适用于老年牙周炎患者的口腔自我管理方案； （3）探讨基于ITHBC口腔自我管理方案对提高老年牙周炎患者自我管理水平的效果； （4）通过方案的实施，提高老年牙周炎患者口腔自我管理的水平和对口腔健康和卫生行为的认知，建立良好的口腔健康行为，促进和维护老年牙周炎患者的口腔健康，提高生活质量。  

Objectives of Study：

By studying the current situation and influencing factors of oral self-management among elderly patients with periodontal disease, combined with literature review analysis, a self-management plan for elderly patients with periodontal disease based on ITHBC was constructed and verified. This provides a theoretical basis for oral health management of the elderly in China, improves and promotes the oral health of elderly patients, enhances their quality of life, and responds to the national "elderly and children" policy. It accelerates the construction of the "pillars" of oral prevention, builds a solid grassroots oral prevention network, and ensures oral "common health and common prosperity". To achieve lifelong management and provide a reference basis for a healthy China. (1) Through questionnaire surveys and baseline examinations, understand the oral hygiene status, knowledge of oral health, and self-management level of elderly patients with periodontal disease; (2) This study is guided by ITHBC, and a self-management plan for elderly patients with periodontal disease is constructed; (3) Explore the effect of the ITHBC-based oral self-management plan on improving the self-management level of elderly patients with periodontal disease; (4) Through the implementation of the plan, improve the level of oral self-management and the awareness of oral health and hygiene behavior of elderly patients with periodontal disease, establish good oral health behaviors, promote and maintain t

纳入标准：

1.符合牙周炎诊断标准； 2.年龄≥60岁； 3.具有完整的认知能力及语言表达能力，生活能自理； 4.知情同意，自愿参加本研究。

Inclusion criteria

1. Meets the diagnostic criteria for periodontitis; 2. Age >= 60 years old; 3. Has complete cognitive abilities and language expression skills, and can take care of daily life independently. 4. Informed consent, voluntary participation in this study.

排除标准：

1.认知或交流障碍； 2.不愿提供电话号码供联络； 3.合并严重全身性和恶性疾病； 4.具有精神症状或有精神病史。

Exclusion criteria：

1. Cognitive or communication disorders; 2. Not willing to provide a phone number for contact; 3. Combining severe systemic and malignant diseases; 4. Having mental symptoms or a history of mental illness.
