研究目的：

本研究计划明确口腔鳞癌细胞中RRAD 基因失活与肿瘤前沿区乳酸代谢的联系，进而证实存在高乳酸环境调控TAMs 中ARG1 蛋白表达上调、从而造成CD8+T 细胞耗竭的分子路径。揭示高乳酸环境诱导TAMs 中ARG1 表达上调的组蛋白乳酸化修饰位点、调控乳酸化修饰的关键分子及相关ARG1 转录因子。验证“RRAD 突变-乳酸堆积-TAMs 组蛋白乳酸化修饰-T 细胞耗竭”链条中标志物和代谢物预后价值；探索抗PD-1 联合葡萄糖摄取抑制剂及精氨酸补充三联疗法应用的可行性。  

Objectives of Study：

This research proposal aims to elucidate the link between RRAD gene inactivation in oral squamous cell carcinoma and lactate metabolism in the tumor-front region, thereby confirming the existence of a molecular pathway through which a high-lactate environment upregulates ARG1 protein expression in tumor-associated macrophages (TAMs), leading to CD8+ T cell exhaustion. It seeks to uncover the histone lactylation modification sites responsible for ARG1 upregulation in TAMs under high-lactate conditions, identify key regulators of lactylation modification, and determine the relevant ARG1 transcription factors. The study will also validate the prognostic value of markers and metabolites within the "RRAD mutation–lactate accumulation–TAM histone lactylation–T cell exhaustion" axis, and explore the feasibility of a triple-combination therapy involving anti-PD-1 agents, glucose uptake inhibitors, and arginine supplementation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄：≥18岁。 2.性别：不限。 3.病变部位：舌体、颊黏膜、上下牙龈、口底、硬腭黏膜。 4.需术前病理活检证实为鳞状细胞癌的原发患者，术前临床分期均为cT1-4N0-3M0(AJCC 8th版)。 5.全身情况：通过常规检查，如三大常规、血生化检查、心电图等，排除手术禁忌证，且通过X线胸片和/或腹部B超和/或增强CT和/或MRI和/或PET/CT等检查排除远处转移。

Inclusion criteria

1. Age: >=18 years old. 2. Gender: no restriction. 3. Lesion sites: tongue, buccal mucosa, upper and lower gums, floor of the mouth, hard palate mucosa. 4. Patients must be primary cases confirmed as squamous cell carcinoma by preoperative pathological biopsy, with preoperative clinical staging of cT1-4N0-3M0 (AJCC 8th edition). 5. General condition: Through routine examinations, such as complete blood count, blood biochemistry tests, electrocardiogram, etc., surgical contraindications are excluded, and distant metastasis is ruled out by chest X-ray and/or abdominal ultrasound and/or contrast-enhanced CT and/or MRI and/or PET/CT.

排除标准：

1.完整的同病情相关临床及基线资料无法获取。 2.我院住院前外院已经行根治为目的手术、放化疗的患者。 3.孕妇或哺乳期女性。 4.存在精神、智力障碍及认知困难者。

Exclusion criteria：

1. Complete clinical and baseline data related to the disease are not available. 2. Patients who have already undergone radical surgery, radiotherapy, or chemotherapy at another hospital before being admitted to our hospital. 3. Pregnant or breastfeeding women. 4. Individuals with mental, intellectual disabilities, or cognitive difficulties.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.本试验设有实验记录本及病例报告表，在实验记录本上记录本研究中实验过程和实验结果以及招募过程中受试者参与研究意愿，病例报告表记录相关内容。所有数据将由课题负责人冯芝恩及时、真实、准确、完整的记录在实验记录本和病例报告表内，定期检查，并由课题负责人负责管理。 2.电子数据文件，如：数据库、检查程序、分析程序、分析结果、编码本和说明文件等，将分类保存，双备份保存于不同磁盘或记录介质上，由冯芝恩医师妥善保存，定期检查，防止损坏。 3.标本配送由课题组专人负责，过程中隐去能够识别患者个人信息的敏感信息，避免受试者隐私信息泄露。4.所有测定结果及其他研究相关纸质版数据妥善保管于常温常湿的上锁柜子中，由冯芝恩医师统一负责保管，定期检查，项目结题后继续保存用于相关科研文章撰写及发表，待两年后文章发表及时销毁，彻底删除相关数据资料。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Experimental record book and case report form were provided for this experiment. The experimental process and results of this study and the willingness of subjects to participate in the study during recruitment were recorded in the experimental record book, and the relevant contents were recorded in the case report form. All data will be timely, true, accurate and complete recorded in the laboratory record book and case report form by Feng Zhien, the project leader, and will be checked regularly and managed by the project leader. 2. Electronic data files, such as databases, inspection procedures, analysis procedures, analysis results, coding books and explanatory files, etc. shall be classified and stored in different disks or recording media with double backups, which shall be properly stored by Dr. Feng Zhien and checked regularly to prevent damage. 3. The research team is responsible for specimen delivery, and sensitive information that can identify patients' personal information is hidden in the process to avoid the disclosure of subjects' privacy information. 4. All results and other research-related paper data shall be kept in a locked cabinet at normal temperature and humidity, and shall be kept by Dr. Feng Zhien for regular inspection. After the conclusion of the project, the paper shall be kept for the writing and publication of relevant scientific research articles, and the relevant data shall be completely deleted after the article is published two years later.
