研究目的：

证明高危结扎与低位结扎对直肠癌的三年无复发生存率（DFS），三年生存率没有差别。  

Objectives of Study：

To prove there is no significant difference between higher and lower vascular ligation in 3-years disease free survival (DFS) and Overall Survival (OS) during TME surgery of rectal cancer.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄 18-70
2. 肠镜：距齿状线<= 12cm
3. 病理证实为腺癌
4. 肿瘤可通过手术进行切除
5. 胸片/胸部CT、腹BUS/腹CT除外远隔转移
6. 无胃肠道手术史
7. 无完全性肠梗阻
8. 无严重心肺功能障碍
9. 预计生存时间超过六个月
10.患者Karnofsky评分大于等于70 11.WBC > 3.5 x 109/ml, PLT> 10 x 109/ml, 总血清胆红素< 22.2 umol/l,BUN <7.14mmol/l, 肌酐< 132umol/l或肌酐清除率> 50ml/min 12.患者必须签署知情同意书
11. 女性受试者参加试验期间必须避孕
12. 绝经妇女至少持续12个月，且排除受孕可能
13.无其他重要相关疾病（例如其他肿瘤，严重心脏和中枢神经系统疾病等）

Inclusion criteria

1. Age between 18 and 70;
2. Colonscopy: tumor below 12cm from dentate line;
3. Pathological diagnosis of adenocarcinoma;
4. Resectable tumor;
5. CXR/CT, abdominal ultrasound/CT exclude metastasis;
6. No history of previous gastrointestinal surgery;
7. Without complete bowel obstruction;
8. Good cardiopulmnoary functions;
9. Expectation survival period more than 6 months;
10. Karnofsky score more than 70;
11. WBC > 3.5*10^9/ml, PLT> 10*10^9/ml, T-Bil< 22.2 umol/l, BUN <7.14mmol/l, Cr< 132umol/l or creatinine clearance rate > 50ml/min.;
12. Female patients in trails must under contraception. And patients who is menopause should be assureed for 12 months and pregnancy should be ruled out;
13. No other relevant diseases (ex: tumors in other sites or serious disease heart or central nervous system, CNS).

排除标准：

1.直肠腺癌／粘液癌之外的直肠良恶性肿瘤
2.不能控制的内科疾病（如不稳定心绞痛）
3. 曾经接受过免疫抑制治疗（包括皮质类固醇）。
4. 既往或同时存在其它恶性肿瘤病史
5. 既往1个月内参加了干预性的临床试验者。
6. 不能耐受全麻或手术治疗。
7. 严重的未控制的感染。
8. 影响认知能力的神经或精神异常，包括中枢神经系统转移。
9. 有严重的过敏史或过敏体质者。
10. 严重肺部或心脏疾病病史者；如急性心梗发作小于6个月、慢性心衰、有症状的心绞痛，凝血功能显著异常等。
11. 孕妇或哺乳及治疗期拒绝避孕的女性；
12. MRI／EBUS／CT提示局部不可切除病变
13. 术前存在直肠癌相关的不全或完全性肠梗阻
14. 遗传性非息肉病性结直肠癌HNPCC或家族性腺瘤息肉病FAP

Exclusion criteria：

1. Pathological diagnosis other than adenocarcinoma and mucinous carcinoma;
2. Uncontroled underlying diseases (unstable angina etc);
3. Previous history of immunosuppresive therapy (including corticosteroids therapy);
4. Past history or excisting other malignancy;
5. Difficulty in endure anesthesia and surgery;
6. Serious uncontroled infections;
7. Diseases of CNS or psyche which affect congnition including CNS metastasis;
8. Serious allergy history;
9. Serious pulmonary heart disease for example: acute myocardial infarction shorter than 6 months or congestive heart failure or symptomatic angina, coagulative function abnormal etc.;
10. Female of pregnant and lactation or patients who reject contraceptional therapy during treatment period;
11. MRI/EBUS/CT suggest unresectable tumors;
12. Preexcisting incomplete or complete bowel obstruction related to rectal cancer;
13. Hereditary nonpolyposis colorectal cancer (HNPCC) and familial adenomatous polyposis(FAP).
