研究目的：

探讨全科医学管理对老年心血管代谢性共病患者治疗依从性、生活方式及临床指标的影响，以期为该类患者提供更有效的疾病管理方法。  

Objectives of Study：

To investigate the impact of general practice management on treatment compliance, lifestyle, and clinical indicators in older adults with cardiovascular metabolic comorbidities to provide more effective disease management strategies for this population.

纳入标准：

1.年龄≥65岁； 2.符合《中国2型糖尿病防治指南（2020年版）》中2型糖尿病的诊断标准，且糖尿病病程在1年以上； 3.至少患有以下一种动脉粥样硬化性心血管病（ASCVD）：冠状动脉粥样硬化性心脏病/动脉粥样硬化性缺血性卒中/外周动脉疾病，诊断标准参见《内科学》( 第 8 版)； 4.签署知情同意书。

Inclusion criteria

1. Aged >= 65 years; 2. Compliance with the Guidelines for the Prevention and Treatment of Type 2 Diabetes in China (2020) and meeting the diagnostic criteria for type 2 diabetes,and the course of diabetes is more than 1 year; 3. The presence of at least one of the following atherosclerotic cardiovascular diseases (ASCVDs): coronary atherosclerotic heart disease, atherosclerotic ischemic stroke, or peripheral artery disease, based on the diagnostic criteria outlined in Internal Medicine (8th Edition); 4. Signed informed consent form.

排除标准：

1.存在认知功能障碍的患者； 2.恶性肿瘤晚期患者； 3.不愿意参与研究或拒绝随诊的患者； 4.无法进行正常交流和沟通的患者。

Exclusion criteria：

1. Patients with cognitive impairment; 2. Patients with advanced malignant tumors; 3. Patients unwilling to participate in the study or refusing to undergo follow-up; 4. Patients unable to communicate or interact normally.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究设计并使用了纸质版的病例记录表（Case Record Form, CRF）来收集所有研究数据。数据收集完成后，由两名独立的研究人员采用双人双录入（Double data entry）的方式，将CRF上的数据录入SPSS 25.0软件中建立电子数据库。我们定期对数据库进行逻辑核查与清理，以保证数据的准确性与完整性。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

All study data were collected using paper-based Case Report Forms (CRFs). Following collection, the data were entered into an electronic database established with SPSS (version 25.0). To ensure the accuracy of the data, a double data entry procedure was conducted by two independent researchers. A data manager performed periodic data verification and cleaning for quality control.
